A POT OUT OF ROUT WAS FOUND ON 12-30-01 BY AN EXAMINER BETWEEN 8-10 AM. THE SECONDARY ESCAPEWAY WAS REROUTED AROUND THE POT OUT. THIS POT OUT WAS CHECKED AND FOUND TO BE A FALL ON 12-31-01. TH E FALL IS 70' LONG, 16' WIDE AND 8' HIGH. THE FALL IS BETWEEN1AC AND 2AC IN 2J AT 78+80. DUE TO WEAKENED STRATA.
THE PASSENGER ELEVATOR STOPPED APPROX. 30' FROM THE BOTTOM LANDING. NO PASSENGERS ON BOARD. BREAK IN TRAVELING CONTROL CABLE. RETURN TO SERVICE 600 PM.
1-A LONGWALL FACE AT #126 SHIELD, AN IGNITION OCCURRED ON THE LONGWALL AS THE SHEAR WAS MINING TOWARD THE HEADGATE. THE TAIL SHEAR OPERATOR STATED HE SAW AN ORANGE FLAME COME OVER THE HEADGATE COWL AT ROOF LEVEL. THE FLAME LASTED A FEW SECONDS AND SELF EXTINGUISHED.
THE SHEARER WAS STOPPED AT SHIELD #175 AND FACE LINE STOPPED TO REMOVE A ROCK FROM PAN. WITHIN APPROX 45 SEC AFTER RESTARTING PAN LINE A YELLOW FLAME WAS OBSERVED AT SHIELD 113. INVESTIGATION FOUND A LINK BROKEN ON ONE SIDE OF FACE CHAIN WHICH SHOWED EVIDENCE OF BEING RUBBED ON, THUS CAUSING A HOT CHAIN LINK. FLAMES WERE OF A SHORT DURATION, NO INJURIES OCCURRED.
AN INSECT CAME IN CONTACT WITH AN ELECTRICAL CONTACTOR RESULTING IN THE ELEVATOR BRAKES TO ENGAGE- STOPPING THE ELEVATOR. ELEVATOR WITH NO PERSONS ON IT WAS OUT OF SERVICE FOR MORE THAT 30 MIN UTES UNTIL THE BURNT CONTACTOR COULD BE REPLACED.
A DUST IGNITION OCCURRED ON THE LONGWALL FACE AT THE #52 SHIELD. THE SHEARER OPERATOR OBSERVED A SMALL YELLOW FLAME THAT SELF EXTINGUISHED. HE STATED THE FLAME LASTED FOR LESS THAN 1 SECOND.
A FALL OF ROOF OCCURRED AT THE TAILGATE OF 3T LONGWALK IN 3 AC OF 25 @ 89 + 20. USING 8'RIB BOLTS FOR TRUSSES AND 6' COMBINATION DOUBLE FORK IN CENTER AND 2 ROWS OF 6X6 TIMBERS BETWEEN XC'S. SLIP RUNS THROUGH AREA LOST POWER ON FACE COULD NOT PULL SHIELDS AND LOST THE ROOF.
A POWER SURGE HALTED THE HOIST APPROX. 100' FROMTHE COLLAR. A BLOWN BLOWER MOTOR FUSE & PRIMARY T205 FUSE WAS FOUND. ELEVEN MINERS WERE TRAPPED IN CAGE APPROX. 2 HOURS WHILE TROUBLE-SHOOTING. HOIST BACK IN OPERATION AT 2:50 P.M.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED ALONG GRASSY CREEK MAINS NO. 4 ENTRY BETWEEN SPADS 14523 & 14656 (INTAKE). ALL APPROACHES TO THE FALL AREA WERE DANGERED AND BREAKERED OFF. APPROX. FA LL DIMENSIONS WERE 30' LONG, 20' WIDE, 10' HIGH.
A FALL OF ROOF WAS FOUND BY AN EXAMINER ABOUT P.M. THE FALL WAS LOCATED AT THE MAIN EAST RIGHT RETURN AT 139 XCUT AT THE ACCESS TO NASON #1 RIGHT SEAL.
SHEARER OPERATOR WAS MINING TOWARDS THE TAILGATE AT 99 SHIELD. HE OVBSERVED AN ORANGE FLAME THAT LASTED ~2-3 SECONDS AND SELF EXTINGUISHED.
A FALL OF ROOF OCCURRED AROUND 9RT 4AC BLEEDER ENTRY 1XC INBY 2% POINT. FALL WAS 9' HIGH, 25' WIDE, 40' LONG, SUPPORTED BY 6' DOUBLE LOCK CONVENTIONAL BOLTS. FALL WILL NOT BE CLEANED UP.

A ROOF FALL WAS DISCOVERED AT APPROX 10:50 AM BY WEEKLY EXAMINER ON THE 4 SOUTH 1ST PANEL BETWEEN #12 AND #13 F BLOCK. THE FALL WAS APPROX 65' BY 14' BY 4 1/2' BY 6 1/2'. TRAVEL OF MINERS OR VENTILATION NOT AFFECTED. NO INJURIES.

A DUST IGNITION OCCURRED IN THE FACE OF #3 ENTRYON THE 2A SECTION. IT WAS REPORTED THAT AN ORANGE FLAME NEAR THE BOTTOM EXISTED FROM 10 TO 30 SECONDS AND SELF EXTINGUISHED. A STRIP OF SULFUR DEPOSIT WAS FOUND AT THE BOTTOM OF THE COAL SEEM, NO INJURIES.
THE PERSONNEL ELEVATOR STOPPED ABOUT 3/4 THE WAYUP THE SHAFT DUE TO A DOOR SWITCH MOUNTING BRACKET BREAKING, DISABLING THE ELEVATOR. 24 PERSONS WERE STRANDED ON THIS ELEVATOR FOR 1 HOUR 10 MIN UNTIL IT COULD BE BROUGHT TO THE SURFACE.

THE SERVICE SHAFT ELEVATOR HOIST WENT DOWN AT APPROX. 10:00 PM WHEN THE POWER CABLE FOR THE ON-BOARD CONTROL BOX BROKE LOOSE FROM THE BOTTOM OF THE ELEVATOR. NO ONE WAS ON THE CAGE & NO INJURI ES RESULTED. CAGE WAS AT THE SURFACE LEVEL WHEN POWER WAS LOST.
THE MIRACLE RUN PASSENGER ELEVATOR WAS DESCENDING THE SHAFT WHEN IT SUDDENLY STOPPED. ONE PASSENGER IN ELEVATOR OUT BY 11:10PM.
ELEVATOR STOPPED SUDDENLY WHILE TRAVELING DOWN THE SHAFT. NO ONE WAS IN THE ELEVATOR AT THIS TIME. THE ELEVATOR WAS PUT BACK IN SERVICE AT 5:45PM SAME DAY.

EE WAS ATTEMPTING TO HOOK UP A SCREEN DECK TO BE LIFTED WHEN ANOTHER EE SHIFTED THE DECK CAUSING HIS FINGER TO BE CAUGHT BETWEEN THE DECKS.

EE WAS RE-RAILING A TRACK PERSONNEL CARRIER BY PULLING IT WITH A SECOND VEHICLE. A POWER CABLE HUNG ON THE MINE RIB WAS LOOPED DOWN AND CAME INCONTACT WITH HIS RIGHT FOREARM AGGREVATING A PRE VIOUS INJURY WHICH HE EXPERIENCED OFF THE JOB 2 MONTHS EARLIER.
EE WAS STANDING NEAR A FLAT CAR LOADED WITH 60"" 500' OF CONVEYOR BELTING THAT WAS HOISTED UP ON ITS END IN ORDER. TO PLACE IT ON THE SUPPLY CAGE ELEVATOR. ONE OF THE CHAINS BEING USED TO SECUR E SEVERAL OF THE LAYERS OF BELTING ONTO THE CAR BROKE CAUSING APPROX. 40' OF THE BELTING TO SLIDE DOWN OFF THE CAR STRIKING THE EE AND KNOCKING HIM DOWN.
2A LONGWALL, EMPLOYEE WAS INSTALLING RAM JACK AT #89 SHIELD. WAS USING CHAIN TUGGETS TO HELP POSITION JACK. HOOK ON TUGGETS CAME LOOSE STRIKING HIM IN LEFT THIGH.
3 EAST MAINS #3 ENTRY- DURRING THE CHANGE OUT OF A SHUTTLE CAR TIRE: TIRE ON CAR NEEDED JACKED UP A COUPLE INCHES TO ACCOMADATE THE NEW TIRE. DURRING THE JACKING PROCESS THE JACK BAR SPRANG UP AND STRUCK HIM ON THE RIGHT SIDE OF FACE.

WAS SERVICING MACHINERY WHEN HIS ARM, CHEST, HAND & LEGS WERE EXPOSED TO ETHYLENE GLYCOL. AFTER WASHING OFF ALL BUT HIS LEGS, THE LEGS DEVELOPED AN INFLAMMATION AND SCALING/PEELING LATER NEXT DAY.
EMPLOYEE WAS WALKING FROM HIS CAR TO THE MINE BUILDING PRIOR TO THE START OF HIS SHIFT. AS HE STEPPED UP ON THE SIDEWALK HE WXPERIENCED PAIN IN HIS LEFT KNEE. CLAIMS HE TWISTED HIS KNEE.
EMPLOYEE WAS LIFTING REBAR AND SUFFERED AN INGUINAL HERNIA.

THE INJURED EE WAS ROTATING THE 2J LW TG DRUM BY HAND WHEN HE WAS STRUCK ON THE LT. HAND BY A PIECE OF FALLING ROCK. SUFFERED FRACTURE TO LT. THUMB & INDEX FINGER.
EE WAS WALKING ALONG 5R TRACK WHEN HE FELT A PAIN ON THE OUTER EDGE OF HIS LEFT FOOT. DIAGNOSED BY X-RAY AS A HAIRLINE CRACK FRACTURE.
4 SOUTH SECTION #9 ENTRY, A PIECE OF ROOF ROCK FELL AND STRUCK EE ON TOP OF HARD HAT AND TOP OF RIGHT SHOULDER, EE WAS EXAMINED, RETIRNED TO WORK NEXT SHIFT, CONTINUED TO WORK, HAD FOLLOW UP V ISIT ON 7-31-01. AT THIS TIME HE WAS INFORMED THAT A BONE IN HIS RIGHT SHOULDER HAD BEEN FRACTURED.
EE WAS BENT OVER PREPARING TO PICK UP ONE END OFA VENT TUBE. CO-WORKER DROPPED THE OPPOSITE END OF TUBE CAUSING IT TO BOUNCE OFF OF PAVEMENT ANDSTRIKE INJURED UNDER THE CHIN.
EMPLOYEE WAS USING A PULL ROPE AND COME-ALONG TO POSITION A TAKE UP ROLLER BACK INTO POSITION ON AN OVERHEAD BELT SYSTEM. HAD LT HAND POSITIONED ABOVE PULL ROPE ON BEAM, TIGHTENING COME-ALONG WHEN PULL ROPE MOVED UPWARD REPOSITIONING ITSELF CATCHING LEFT HAND FINGERS - RING, MIDDLE & POINTER.

AS EE REACHED TO TURN THE WATER ON AT THE LONGWAL (1ST RIGHT) HEADGATE A BUMP OCCURRED CAUSING A ROCK TO FALL CATCHING HIS LEFT WRIST AND HAND. THE INJURY RESULTED IN A LACERATION AND FRACTURE OF THE LEFT FOREARM (WRIST).
