Roof fall on #8 belt 33 blk 6-7 crosscut area 15'w x 40'l x 9'h.
An unintentional roof fall was discovered at approximately 10:30 p.m. on the 3 East Mains at Crosscut No.52 in the No. 2 bdg. The fall was in the intersection. The fall was approximately 6'-8' high and 20' wide and approximately 10' long. All approaches to the fall were timbered off. The fall did not interupt ventilation.
A roof fall was reported on 12/01/2011. After further investigation of dangered off areas 5 falls were reported. All five falls had been dangered off with a physical barrier and were assumed to be inactive areas.
After investigation it was determined that a roof fall occurred on the mainline track in a crosscut at 336+00. The actual time of the fall is undetermined. The fall was 26 feet wide 15 feet high and the length was undetermined due to access.
Unintended roof fall was discovered at approx 2:35 am on the 3 east mains at crosscut No. 11 in No.4 hdg. Fall was in intersection. Fall was approx 10'high and 20'wide and approx 30'long. Fall caught high voltage cable and cable had to be recruited around the fall. All approaches to the fall were timbered off. The fall did not interrupt ventilation and will not be cleaned up.
While traveling in the mine a roof fall was found in the croc cut # 20 between #3 and # 4 entries in the 2nd Right section. This fall is a continuation of an area that had been cribbed and dangered off and later fell. The new area of the fall is 20'W x 20' L x 6' H and has fallen over a return/intake stopping and a single row of timbers.
The portal passenger elevator went down at approximately 7:05 AM. Schindler Elevator Service was called to evaluate the occurrence and correct the problem. The elevator was returned to service at approximately 10:30 AM. A defective door strut, that allows the elevator doors to shut, was found to be the problem.
The Kulntown elevator went down with 15 persons on board. The people were manually brought to the surface and released at 4:27pm. The problem was electrical with the encoder. Elevator back in service 6:15pm.
The ropes in the counterweight sheave jumped out of the sheave due to a bad sheave bearing.
Kuhmtown elevator went down at 8:04am with 20 personal on board. People were lowered to the bottom and safely released. Rope dogs actuated and encoder. Elevator back in service at 10:10am. Encoder changed and speed adjusted.
An unintentional roof fall was discovered at approximately 1:00pm in the left return on Grassy Creek Mains, at crosscut No. 13 adjacent to survey station No. 138. The fall was approximately 8'high, 20'wide, and approximately 20' long. All approaches to the fall were timbered off. The fall did not interrupt ventilation and will not be cleaned up.
The Kuhntown elevator went down with 6 persons onboard. Elevator was lowered to the bottom location and persons were released (5 min). Problem was bad end coadu. Elevator back in service 5:00 am.
The Kulntown elevator went down at 8:22 am with persons onboard. Elevator was lowered in manual and persons were released. The rope dogs activated. The switch was replaced. Elevator back in service 12:10 pm.
25 people were entrapped on the bottomed landing of Metz elevator, for 2 hrs 45 min.
Roof fall occurred 10/11/11 on the main East #11 Belt at the 11-30 track to belt cross cut. The fall is approx. 80' L.X 21"" W. X 15' H.
The 5 north escapeway hoist was removed from service due to a bad limit switch on the hoist. Portable hoist truck placed on site. Hoist was repaired by tensioning chain on 10/7/11 at 10:40 A.M.
The wana service elevator went down due to damage to the bottom beams and alignment to the counter weight guides. Repairs were made to the elevator and service resumed on 9/22/11 at 6:40 P.M.
The wana elevator went down at 10:00 am with no persons on board. The elevator was back in service at 10:47 am. Elevator problem with main breaker was the problem.
Thirteen employees got trapped on the portal passenger elevator at approximately 12:53 am. Schindler Elevator Service was called to evaluate the occurrence and correct the problem. The employees were freed from the elevator at approximately 1:40 am. A defective door switch on the bottom door landing was found to be the problem.
A fall was discovered at Main North #1 Entry between 167-168 block. The fall is between 1219 and 8803, and is 20' long/5'height. No persons affected, ventilation not disrupted, walkway is obstructed. Area dangered off. Face will be bolted and walkway reestablished.
Entrapment of 7 people on the Metz elevator due to power loss.
An unintentional roof fall was discovered at approximately 12:30 p.m. between the no. 1 and no. 2 entries on the 3 East Mains at break No. 79. All approaches to the fall were timbered off. The fall did not interrupt ventilation and will not be cleaned up.
The wana skip hoist elevator went down at 10:30 am with a person on board. The person was safely removed from the elevator at 12:35 pm. electrical problem in the trailing cable, restraining clamp slipped and pulled cable in the control box.
A roof fall occurred on the old main line track at survey spad 304+00. The fall was approx 35ft long 15ft high and 18ft wide.
The WANA Service Elevator went down at 8:30 AM with no persons on board. The elevator was back in service at 12:00 PM. The problem was electrical in the control cable.
A roof fall occurred on the mainline track at 66+00. The fall was approx 30 feet long, 10 feet high and 16 feet wide.
A roof fall was discovered at 24.5 block on the 6 North track haulage at approx. 10:40 p.m. This fall is approx. 5' H. X 10.5' H. X 10.5' W. X 14' L. Fall will be bolted and cleaned and screened.
Whittaker elevator had damage to the door seal at the mine level and repairs had to be made.
A fall was discovered at 11-M Suals crosscut #1 to #2 between spad locations 4351 to 4411 which is an outby area. Ventilation was not disturbed and this area was dangered off. The walkway is obstructed, Sual can be examined.
The wana service elevator was damaged while hoisting a scoop cay. The hook broke free arrowing the scoop to fall causing damage to the side wall, floor of elevator and winch.
Following a routine maintenance by Schindler Elevator service, 2 employees were briefly entrapped on the mine' s passenger elevator. The elevator was lowered to the bottom where the employees got off. Schindler Elevator service was recalled and found and replaced a defective governor rope slack switch. The elevator was returned to service at approximately 6:15 PM.
While mining from the tail to the head, an alleged ignition occurred while checking the head coal thickness at the head shearer drum located near 195 shield on the 18A panel
Roof fall in primary escape way, 18'l X 15'w X 12'h in the #1 heading of 16D at 34 Blk.
Two employees got trapped on the elevator at approximately 5:11 AM. Schindler Elevator Service was called to evaluate the occurrence and correct the problem. The employees were freed at approximately 6:15 AM. The elevator was throughly examined and evaluated. The elevator was returned to normal service at 10:30 AM.
An employee got trapped on the portal passenger elevator at approximately 9:44 AM. Schindler Elevator Service was called to evaluate the occurrence and correct the problem. The employee was freed and the elevator was returned to service at approximately 2:20 PM. A defective slow down switch approximately 80 ft. from the top landing was found to be the problem.
A roof fall occurred on the mainline track at survey station 16-00. The fall occurred above the anchor point. The fall was approx. 30' L.X 16' W. X 9' above the coal seam.
A cc located at 44 blk, 6 North between an int. air course and belt had been previously dangered off due to 2 potted out areas along each rib. The area is between a arch support and a permanently dangered off stopping. There is no reason why anyone should have to travel this area. The potted out areas have now further deteriorated to the point that we feel it may be reportable.
A leak in the J tube of the cooling line allowed a section of fire wrap to ignite. The small flame was immediately extinguished. The flame reappeared and was successfully extinguished with water. The flame was contained to within the confines of the motor compartment. No individuals were injured. No area of the mine was damaged. Total time did not exceed 10 - 15 minutes.
Miner oper. had taken a cut of coal out of face of No.3 hdg. that was approx 15' in depth. After the coal had been removed, a section of mine roof in unsupported area of newly mined cut fell. Size of section of roof that fell was approx 14' wide, 12' long, 0-2.5' thick. The material was sandstone. The ignition occurred when miner oper. started to cut the material. Put out 3-5 mins.
At approx 7:40pm, a haulage accident occurred on the surface area of mine. The EE was in the process of attaching a wire nip to a trolley pole of a ditch digger while apparently standing in front of the ditch digger. The ditch digger became energized and contacted the EE causing injury and subsequent amputation of the lower right extremity at the hip. EE was pronounced dead.
He was laying rail in the 15a section. When lifting a rail his right middle finger was caught in between the rail he was lifting and a stationary rail, resulting in an amputation of the tip of the right middle finger.
Employee was struck on top of the right foot while using a 3 ton tugit to lift a coul-arm over the handrail. The hook-up slipped causing the coul-arm to strike his foot.
Employee was unloading hay from truck when he fell off back of 1 ton dump truck 9 ft up. Left leg & right shoulder.
He was in the process of closing the rib protector door on the miner and had his left hand on the rib pinner. When the door shut. It caught his left middle finger between the rib protector door and rib pinner. This caused a fracture/laceration to the tip of his left middle finger.
He was in the process of working on a shuttle car, watching the cable reel wind up. The cable struck his feet, causing him to fall backwards, striking his head on the ground. This resulted in a laceration to back of his head.
While attempting to get on top the #3 CMI to perform maintenance on the upper discharge fins, the employee lost his balance, slipped and fell to the floor. The resultant injury was a fracture to the right hip.
Individual was assisting in moving the Cat Fletcher Bolter. He grabbed onto a rib bolt while stamping on a piece of curtain which was stuck in the cats of the bolter. The bolter moved jerking the curtain out from under his feet causing him to fall. The right hand was holding the rib strap, cutting through the glove causing a laceration on right middle finger.
The injured was unloading belt structure when he felt a ""pop"" in his left knee. The incident became reportable when the injured began missing work on 12/30/2011.
He was installing an end bolt off the miner, when he had to stop to fix the suction on the bolter. As he was bent over, a rock fell from the roof and struck him at the base of the neck which resulted in a fracture of the C6 vertebrae process.
Pumping water at 18 D Transfer when the airline that was hooked to the pump came loose and struck the employee in the right eye.
EE stated he was operating manbus & bus was struck by a tram causing the right side of his body to strike the engine compartment. The EE stated he felt pain in his neck & middle of his back. The alleged injury was reported on 10/23/11 but he did not miss any work until 10/31/11. The alleged injury is being challenged by the Co. & Co reserves the right to remove from record
He was stepping off of a equipment dolly in the supply yard and felt pain in his left heel
Attempting to lower an air drill after a cable bolt was installed when the drill spun around and the handle struck him in the head causing a contusion.
Parked 30 feet inby the feeder in 9 South Mains, went to step out of Ram Car #4 to go shovel the feeder and rolled his right ankle upon dismount. No issue with training, PPE, mining machinery or compliance.
Dragging rock dust hose and struck his head on the roof. Was diagnosed as post concussion syndrome on 10/24/11
Struck his hard hat against a low hanging roof strap while walking resulting in neck/shoulder pain. Starting missing work on 11/4/11 and was diagnosed on 11/29/11 for spinal damage.
Employee experienced a superficial right eye injury when debris went around his safety glasses and into his right eye. Employee was back to work on 10/7/2011. Injury was not reported as it was uncertain at the time that this was a MSHA related claim; after speaking to MSHA it was determined that the injury should have been reported.
#2 Mother belt. Descending stairs and felt pain in left knee. Diagnosed with tear in ligament and meniscus on 10-11-11.
The IE and the hauler operators were attempting to extend the ventilation tube slider, when the roof collapsed. The falling roof hit the tubing which knocked the IE down and then the rock hit the EE on the left foot. The falling material cause laceration to back and laceration and crushing fractures to his lt. foot. Rock size 1' - 3' thick and approx. 20' L. X 8' W.
He was retrieving a come-along off of the discharge end of the stage loader, when a rock passed through the stage loader causing the cover to raise, which struck him in the face.
In the process of installing a bolt, when he pinched his right middle finger between two bolt plates.
Individual was walking along the kulntown rockdust chute when the left foot became caught in a tie down rope causing the individual to trip. The individual fell with the right hand on rail and the left hand on the ground. Individual went off work for surgery to the shoulder - rotator cuff 1/5/12.
Individual was a rib bolter on the 18w section fall #1 entry. The corner of coal pic rolled out striking the right side of face, pinned both legs thighs and right ankle. Individual suffered an abrasion to the right temple area, and bruises to the thigh area.
Individual was carrying a roll of 2"" hose over an o'cast when the hose caught the top of the o'cast wall causing him to twist. Individual felt pain to the left knee.
EE was placing a roof channel on the timber jack of the miner and caught his right index finger in between the timber jack and roof channel causing a laceration to his right index finger.
He was driving #86 jeep past #22 jeep that was switched out. When driving by a scraper pipe on #86 jeep caught on #22 and struck him in the face causing lacerations.
Employee was viewing the #5 locomotives wheel location when the motor unexpectedly dropped off the rail striking the employee's right leg, resulting in compound fractures.
Individual was repairing a shuttle car splice and while cutting the cable felt pain in the right shoulder while handling cable. Worked until 1/12/12 when individual went off for shoulder surgery.
Tripped and fell while walking along side a scoop landing against the rib on his left shoulder. Worked until 2/6/12 and had rotator cuff surgery.
Individual was dropping ventilation tubes when a ventilation tube bounced off the rib and struck the left knee bending the knee backwards. Individual continued to work until 9-20-11 when he had surgery to the left knee.
Individual was walking down 15W track entry when tripping in an off set in the mine floor. Falling on right wrist. Individual continued to work until 8/24/11 when he went off work due to incident.
The injured has exited a scoop when he twisted around to walk and fell resulting in a broken leg
Individual attempting to free a stuck drill steel, had right hand on or in the vicinity of steel. Left hand was on the control levers, apparently on the wrong control putting up pressure on the steel. The steel bent, catching the right hand between the steel and bolter mast. Individual suffered a broken index finger and sutures to the middle finger.
Stated that on 8/9 he was stepping over a relay bar and twisted his right knee. EE started missing work on 9/29, to have surgery to repair knee.
Individual was retrieving a roof strap towards the back of the miner when a piece of corner rib fell striking the outside of the left foot. Individual received a fracture to the fifth metatarsal.
The injured was helping splice the belt after pulling slack out of the take-up. While making the splice the injured repositioned the knocker and another EE was using a hammer to hit the knocker. EE accidentally hit the injured on the left index finger with the hammer causing a laceration that required 6 sutures.
As the employee was attempting to add a section of drill steel, the top drill steel fell out of the roof striking the employee's left gloved ring finger resulting in a laceration and a fractured finger tip.
EE was working on the surface at the prep plant garage. EE was checking loose bolts on the pan and when descending on the ladder EE slipped on the wire rope ladder and fell on the buttocks. Pain in the lower back area.
EE was loading supplies onto the 19-w continuous miner at the face area. When placing a bundle of roof bolts on the miner the bundle of bolts fell and the EE attempted to catch them. EE caught the right hand between the bolts and the rib pinner. Fracture to the right pinky finger palm.
Strained left shoulder while using a pipe wrench to turn a bed rail. Worked until 4/19/12 had rotator cuff surgery.
Individual was walking into 3-N section when stepping over blue 4"" plastic pipe felt a pull in the right knee. individual continued to work until 5-5-12 when he went off for surgery to the right knee.
EE was attempting to clip a tie to a rail when the rail moved and three fingers on his left hand were caught between the track tie and the mine floor. He experienced fractures and lacerations to ring, middle and index fingers.
While splicing a cable, EE received 6 staples above the left knee by a utility knife.
The employee was struck in the rear of the upper left thigh by a section of rib drill steel which sprang free from the rib bolter as the drill steel was being removed from the drill hole. The employee sustained a contusion - hematoma to the left upper thigh.
Employee was assisting another in moving a lw ram jack when he slipped in the mud around #1 shield and his right knee twisted when he planted his right foot. The employee continued to work and the incident became reportable when the employee began missing work on 09/08/2011
In the longwall of 16D, a duckbill scoop operator was backing up and turning. The injured was standing too close and the duckbill attachment caught his right foot between the duckbill and the floor causing a contusion. No issues with training, mining methods, equipment or compliance.
The employee stated that as he was cleaning up a rib roll along the belt, a piece of coal/rock he was handling bounced back off the belt striking the employee in the ring finger of his left gloved hand resulting in a fracture.
He was walking and stepped on a rock and twisted his left knee. He continued to work until he wanted to have an MRI. MRI results showed a ligament tear to the knee. He began missing work on 8/19/11 to have surgery.
The employee was struck by a small section of head coal which fell from permanently supported top. The material struck the employee about the head, neck and shoulders. This event became reportable when the employee began missing work on June 10, 2011 due to complications (mild concussion) resulting from the incident.
Was dragging a gopher drill in a low area of the mine. Felt strain/pull in left shoulder. Original RPI 5/19, surgery on 10/31.
17-W left long-wall set up face, Individual was attempting to reposition a nylon scoop rope when his left hand was caught between the scoop blade and wrench rope. Individual suffered a fracture to the left index finger & middle finger, received 9 sutures.
Individual was working at the 5 West #1 Transfer removing material that accumulated along the belt when he was lifting up material felt pain in lower back. Individual continued to work until 5-17-11 when he went off work.
EE was dragging and hanging 12470 hi-line above the head. EE felt pain between the shoulder blades. EE continued to work until 5/26/11 when he went off work.
Felt neck pain while operating shuttle car. Did not hit head. Was ruled compensable and mine notified on 10/6/11.
EE was track cleaning on 6 North Haulage at 26 block when he attempted to move a rock from the walkway. EE felt pain in right side of abdomen. EE continued to work until 5/16/11 when he went off work.
EE was cleaning the bunker area with a high pressure water hose, the water hose broke at the coupler and the pressure caused the right hand to twist backwards causing pain and swelling to the right wrist. EE cont. to work until 5/26/11 when he went off work not able to lift with right hand.
EE was piecing a compressor nip on the trolley wire when an arc flash occurred the EE jumped back and fell on the ground landing on his right shoulder.
EE was recovering shields in 3B when he pinched his left pinky finger with the chain resulting in a fracture.
The employee was in the process of using a hand drill to repair a plastic guard when his left gloved index finger was caught by the spinning drill bit when it passed through the material. Medical evidence revealed a fracture of the left index finger.
Sitting on #55 Jeep when it was struck by a motor and he experienced a laceration to his face and a fractured knee cap.
EE fractured right forearm when struck by a wheel unit of a cable dolley while raising the wheels for transport.
Prying rails with a bar off a flat car and felt pain in upper back.
The injured was using a portable Z drill to roof bolt on the face of the 15 Right Longwall Panel, when he started to drill into the mine roof, a piece of rock fell out and struck the injured on top of the head. The injured sustained a fracture of the C1 vertebrae of his neck.
Individual was stepping over a 3' wide by 10"" high pile of coal. Individual stepping into a hole in the pavement felt pop in the right knee and knee extended backwards.
Individual was moving belt rollers from the walkway. When walking thru the X-cut the individual stepped on a rock and twisted the left ankle
EE was pulling back the long wall face conveyor when he pinched his left foot between the shield pontoon and the relay bar. EE experienced a fractured big toe and a laceration.
Injured miner was rockdusting at 55 block on 10 belt. Miner dropped rockdust hose on the floor and felt pain in his lower back while walking from the belt to the track. The miner was treated with physical therapy. No issue with training, mining equipment, methods or compliance.
Miner was in the west end of the Miracle Run Motorbarn taking off a hydraulic motor for the fan on a 15 man diesel mantrip. He stepped up on a stool to reach the hydraulic motor and felt a pop in his right knee. No issues with training, mining equipment, methods or compliance.
Individual was carrying crib blocks and tossing them in a pile when the individual felt pain in the right shoulder. Individual continued to work until 4/8/11 when ee could no longer work.
Mantrip ran into a spur and struck a parked mantrip causing low back pain to passenger.
Individual was lifting a ventilation tube and felt pain in the right shoulder and arm. Individual continued to work until 6/28/11, then went off work due to injury.
Due to medical condition the EE was dizzy and lost his balance and fell in the locker room. This fall resulted in a fracture to his left arm.
Individual was walking along the section power center when he tripped on a loading machine cable lying on the mine floor. The left foot caught the cable and the right foot rolled and twisted when attempting to regain balance when falling against rib and floor, sprained ankle.
Employee fell and landed on his left ribs against a small rock while positioning an exhaust fan bologna skin, resulting in a fracture of his 11th rib.
Individual was carrying belt vulcanize beams to the 6 north belt take-up area when he stepped into a hole in the paulmont twisting the right knee. Individual continued to work until 4/27/11 then went off for surgery to the right knee.
Loosening bolts on feeder when he felt pain in his right arm. Late report 1/28.
EE was working at the face in 18 W-40 blk, turned to pick up one end of a 10' X 20"" ventilation tube to hand to another EE when pain was felt in his lower back.
Employee was removing relay bar pins on the 15D longwall tear down using a hammer and felt pain in his right shoulder. Employee was diagnosed with a rotator cuff tear. No problems with mining methods, machinery, training, PPE or compliance issues.
EE was walking to the front of the loader when he stepped on the edge of a tire rut causing him to slip and fall, this resulted in a fracture to his left hip.
The employee stated that while hanging miner cable with 7 other employees, he injured his left shoulder. The incident became reportable on 3-5-2011 when the employee began missing work.
Fell while inspecting #2 fresh water impoundment resulting in a fracture of the left radius.
The incident occurred on 5 NO mains. The ee was installing 18"" belt hangers. Had drilled the hole and placed the paddle wrench in the drill pot. Turned to pick up an 18"" RB when he hit the rotation on the RB. Paddle wrench rotating in the drill pot when the injured turned into the rotating wrench, striking him in the Rt cheek. The ee sustained fractured to both cheek bones.
Employee was walking belts doing his weekly exams. He slipped causing his right leg to slide forward. His left leg folded under him putting his full body weight on it. He felt pain is his left knee and heard a pop.
While a section of 8 inch casing was being hoisted at 4 foot off the ground, the employee was holding the tag line attachment of the pick up line, the weight shifted and pulled him forward. His left foot was stuck in 2 inches of mud, he lost balance and stumbled forward. He believes that this is when his injury occurred.
Unknown, accident was not reported by the employee
Pinched right pinky finger between pie pan and pie pan bracket. result was 10 stitches.
The injured was installing a roof bolt in the brow of an overcast on 5 North Mains when a piece of rock was dislodged from the overcast. The rock struck the injured in the face, causing a laceration that required 8 sutures in the right cheek, and a small puncture hole through the bottom lip.
A scoop traveling inby in the 3 Left heading of 17D that went into a water hole and got tangled up in ropes and cables underneath the water. This caused the loader cable to be pulled tight and come off the hangers and strike the injured in the right arm and the head. Employee received contusions and was put on restricted duty. No training, machine, mining or compliance issues.
The injured was working on 5 North in the No.5 heading. He was establishing ventilation and while picking up a 4"" X 4"" post he felt pain in the middle of the lower back. 7000-1 is submitted to abate citation No. 8190871 and the mine operator reserves the right to expunge the record if conference-litigation or court decision is favorable to the mine operator.
EE was working at Robinson Run No. 95, EE was cutting stainless steel flat bar with a chop saw. EE started to cut the saw bounced back striking his left knee. EE was taken for medical treatment for laceration to his knee and received sutures. The sutures will be removed 14 days from 7/4/11. EE is now working light duty at 301 slab fork road, slab fork WV.
Injured was in Metz Motorbarn disconnecting the negative blue battery lead from E motor to replace the plug when the wrench he was using contacted the frame and shorted the battery out, resulting in 2nd and 3rd degree burns to his left hand. No issues with training, equipment, mining methods or compliance.
EE was pumping cribs #3 entry at break 36 when the hoses lost pressure. It was discovered the cement line blew apart in #4 entry at break 35. The surface was contacted and instructed to reduce the flow rate. EE tried to reconnect the lines with material still flowing but could not. The line blew in a waterhole which filled with material soaking his boots.
While walking, the employee's left ear caught an edge of a section of roof wire screen matting resulting in a laceration to the upper portion of his left ear requiring 7 sutures.
Pinched end of right middle finger between a red-eye bolt and the tightener resulting in a fracture. Initial x-ray was negative, x-ray on 12-29-11 indicated a fracture.
He was in the process of trying to remove a bolt that stuck in the roof. When the bolt fell out it pinched his left index finger between the bolt and the bolt wrench. this resulted in a laceration to the left index finger.
While assisting another mechanic - electrician indicated that his right hand slipped over the cable and into an open razor knife, that was in the hand of the other mechanic. Electrician was wearing gloves. He received 7 sutures to the hand.
He was standing on the 4B face observing shields being pulled when a rock rolled out between shields striking him on his left index finger causing a laceration.
He was on the BB LW setup, attempting to connect the relay to the panline. When moving relay bar, the chain hoist slipped striking him in the head and hand.
Hosing second floor of prep plant when he tripped and fell landing on right arm and shoulder. He was injured on 9/17/11. Determined that he suffered a fracture to his humerus bone to right arm on 9/23/11.
Bolting off the right side of the miner in 19D when the bolt bent forcing his hand against the mast of the Joy miner bolting station. Received contusion to index and middle finger and fingers were splinted together temporarily. No issues with PPE, training, compliance or mining machinery. Poor hand placement was a issue.
Traveling 9 South Haulage from 11D to 12D, operating the tail motor of a rock dust motor crew, the injured employee struck her hard hat on a low hanging roof strap suffering a sprain/strain to her neck. The miner missed no work, but was treated by a physical therapist. No issues with training, machinery, mining methods or compliance.
EE come into contact with drilling mud. Ph had spiked in the mud. EE was cleaning a mud spill caused by a plugged screen. EE had sustained miner burns and was treated with vinegar & bicarbonate. Subsequent contact with drilling mud irritated the inflamed area. EE left the jobsite saying he refused to work in that environment and sought med treatment on his own.
The employee stated that as he was walking from the train loadout control room to an outside landing, he tripped and fell over a section of conduit laying in the walkway. The resultant injury was a laceration to the upper lip and the left knee, each requiring 3 sutures.
EE was operating #4 Jeep on main north haulage road at 152 block. The Jeep struck a broken rail causing the Jeep to halt and come off the track. EE struck the left side of the face and nose against the frame of the jeep. EE suffered a nasal fracture to the cartilage of the nose.

The employee was in the process of cutting belt with a razor knife. The knife slipped and the employee struck his right leg above the knee, which resulted in a laceration.
The employee stated that he was struck in the upper right facial cheek by a loose end of a flexible ventilation tube. The side shield of his eyeglasses appear to have caused a laceration to his cheek. The resultant laceration requires 6 sutures.

Cleaning Miracle Run bottom area with a wheelbarrow. Injured started to push the wheelbarrow across the track rails when the wheel got stuck and it started to dump. Injured tried to steady it and felt pain in the lower right side of his back. Miner missed no work, was treated by chiropractor. No issue with training, equipment, mining methods or compliance.
Individual was roof bolting on the left side of the miner on 17 W Section #1 Entry 83 block. When attempting to make sure the dust box was sealed by rotating the drill head the starter steel which was in the drill chuck at the base and the top of the steel struck the individual in the lower lip & jaw area. Sutures were installed inner & outer lower lip.

Injured was sitting in mantrip on way to Metz bottom. Driving down two track, Metz bottom, and ran into 5 spur striking another parked jeep. The switch was left thrown for the turn. Employee felt pain in back and was treated with physical therapy.

Individual was bolting on the alpine miner on the left side when a small piece of rock (4""x4"") fell striking individual on the upper lip. Individual received 3 stitches.
EE was traveling down a pair of steps when he caught his left heel on a step causing the EE to fall forward striking the left side of his face and left eye against a fire barrel located at the bottom of the steps.
Operator was splashed with boiling water when flush hose was removed. Burns sustained to 20% of operators body
Employee was attempting to open the door on the O panel section overcast, when the front wall where the airlock was located collapsed and caught the left hand across the knuckles at the ring and little fingers. the left little finger was fractured.
The mine's safety department was notified on 10/3/2011 that the employee was diagnosed with an occupational illness (Bilateral Carpal Tunnel Syndrome).
Was diagnosed with 1.4% noise induced hearing loss. EE was notified on April 25, 2011.
Individual was seen by doctor and was diagnosed with carpal tunnel syndrome to the wrists.
Was diagnosed with 3.66% noise induced hearing loss. Loveridge was notified on 3/9/11.
Medical evidence was received on 2-18-2011 that this employee had received an award of 4 % for permanent impairment, resulting from noise-induced hearing loss.
Was diagnosed with 10% Occupational Pneumoconiosis, Loveridge was notified on 2/15/11.
