A non-injury accident, the unintentional stopping of the Camp Run Portal elevator. The source of the malfunction was found to be with the top door hatch switch. The elevator was returned to service at approximately 12:54 PM. Schindler Elevator Service was call to evaulate the condition.

A hoisting accident occurred at the Contrary Portal. A large piece of ice fell from the shaft wall striking the personnel hoist cage. The ice caused the canopy to shift off of it's supports. The canopy got caught on the landing when it was brought to the top of the shaft.
A roof fall occurred on the 17w section at 12:20pm at 76 block inby the belt tail pieces. No injuries were reported. Ventilation to the #1 face was obstructed and corrected. The fall will be bolted with rod bolts and cleaned up. A joy shuttle car is trapped under brow fall and will be removed.
Emery Mine experienced an intentional fall in the pillar line (GOB) on 11/22/2010 MSHA has requested that we file a 7000-1 report for this incident, we do not agree that this is a reportable accident or immediately reportable.
A fireboss during a preshift examination found a roof fall measuring 100' L X 20' W X 6' H in the 5th West 8th South connection # 1 x-cut # 3 entry. The roof fall did not effect escapeways or air currents. The area was bolted with 5' combination bolts.
The service elevator went down due to a stuck contactor. No persons on board at 2:05 p.m. Elevator repaired at 3:07 p.m. Repaired contactor.
A roof fall occurred on the old 4 west mains at 23 block cross cut 8 to 9. The fall did not disrupt ventilation of the walk way. No people were affected by the fall. The fall area was dangered off and the brawls supported.

17W Section, #3 Entry CBM Well BH-2A (please note that this 7000-1 form is being filed for reporting compliance purposes only, as we do not feel that an inundation occurred). BH-2A was intersected and the approved plan was followed. The hole was grouted to plan and mining resumed.
Roof fall discovered at 75 Brk #3 Intersection at St Zero return aircourse, blocking travel of weekly examiner.
The Kuhntown elevator went down at 7:45am at the top of the shaft. There were no persons on board. The door switch was broken and replaced by schindler at 9:30am.

The source of the malfunction was found to be in a set of contacts in a bumper switch. The elevator was returned to service at approximately 9:05 AM. Schindler Elevator Service was called to evaulate the occurrence.
A fall occurred at 13-M #1 entry at D5622, ventilation to the #1 seal is obstructed as is the walk way. Fall will be re-supported by post 1 cribs to re-establish the walkway and vent to the seal. No persons affected by fall.
A roof fall occurred on the mainline track at survey station 32+00. The fall is approx. 25 ft. long by 18 ft. wide and 10 ft. high.
Roof fall occurred on the 6 North belt at 23 block intersection. Ventilation was not disrupted, walkway was obstructed. The fall area was dangered off on all approaches. This area will be cleaned and supported with bolts and arch supports. No person affected by the fall.
No accident - we had a unplanned rock fall - no one was affected and the ventilation was not affected - rock fall blocked travelway. This was reported immediately after getting outside
An unintentional roof fall occurred on the 3 East Mains in the No.6 heading at corsscut No.49 Spad No. 20747. The fall measured 30' long X 25' wide X 15' high. There were no injuries, no equipment was caught, and the ventilation was not disturbed. This fall will not be cleaned up.
A roof fall occurred on the mainline track at Survey Station 149+50. The fall is approx. 20' long and 30' wide, including the intersection.
A fall occurred at a previously canopy area of the main north bcct at 216 block. Ventilation nor belt opciation were disrupted. Area around fall supported and dangered off. Area will be rebolted.
EE was in the elevator coming to the surface when the elevator stopped approx. 5' from the top. The EE was safely removed from the elevator at 4:35 a.m. by Schindler elevator service. The problem was a bad contactor.
Two miners were trapped for 1 hour and 20 minutes on the Miracle Run elevator due to a communication cable intangling with the slow down limit cam.
Roof fall above the anchorage in #8 heading 173 BLK 3 way intersection of 9 south. 30'l x 20'w x 10' high area was bolted with 8' bolts.
An unintentional roof fall occurred on the 10 right section at break 86 between No 1 and 2 heading. The fall measured 10 feet high by 20 feet long and 18 feet wide. There were no injuries, no equipment was caught and the ventilation was not disturbed. This fall will not be cleaned up.
A fall was found in the 8-S area, CC #2 & #3 which was in a outby area of the mine. Ventilation was not disrupted and this area has been dangered off. All approches supported. The fall will be supported to restore travel way.
Roof fall 30'L X 26'W X 12'H in 37 blk main west belt line intersection.
ONe miner was trapped for 45 minutes on the Sugar Run elevator due to the door switch needing adjusted.
The Kuhntown elevator went down at 9:15 p.m. with 2 persons on board. The 2 people were removed safely from the elevator at 9:49 p.m. the problem with the elevator was that the contactors did not make-up.
The Kuhntown Elevator went down at 7:05 am with one person inside the elevator. At 12:00 pm the individual was safely removed from the elevator. At 1:55 pm the elevator was fully operational after the run-by on the govenor counterweight was adjusted and the elevator was determined to be safe to return to service.
The Kuhntown elevator went down at 8:20a.m. with no persons on board. The elevator was back in service at 12:00 p.m. The problem was electrical,it had a burned resistor.
The 5-north air shaft hoist went down at 9:15am (mining ceased at this time) Hoist was being tested at this time. The hoist had a low oil switch malfunction. Hoist was repaired at 11:15am mining resumed at this time.
On 5/8/10 the 17 W. section #3 entry experienced an inundation of Methane in excess of 5.%. After intersecting horizontal WCC (BH-3A-ST-2) the power was removed immediately from this area as per cut thru plan. Entry was re-ventilated to correct condition. Section was placed on a J order, then K order to allow mining to resume.
A methane ignition occurred on the 17-w section #3 entry at 6:15 am on 5/8/10. The section was placed on a J order and then modified to a K order to allow temporary roof supports to be installed so packers could be installed on both sides of the vertical-horizontal de-gas holes (well BH-3A st 2).
The Kuhntown elevator went down on 5/1/10 at 7:10am with 1 person on board. The person was safely removed at 5:55pm. The counter weigh bearing was bad the repairs were completed on 5/4/10 at 8:15am. The elevator was inspected and released for normal use buy MSHA and state.
2B Longwall had a fall above the anchorage point located in the tailgate entry of the working section at spad number 59+24 to 58+40.
A non-injury alledged face ignition occurred on the 16A longwall near the #133 shield while mining towards the headgate.
An unintentional roof fall was discovered at the Page Service Shaft bottom in the No 6 entry in the intersection at survey No. 100. The fall measured 40 ft long x 20 ft wide x 7 ft high. No one was injured, the ventilation was not disturbed, and no equipment was caught. This fall will not be cleaned up.
15-W Section long wall face ( Please note that this 7000-01 form is being filed for reporting compliance purposes only. As we do not agree that an inundation occurred) De-gashole 16 WHD - H- 6 was intersected and by following the vent plan the hole was grouted and mining resumed.
Three miners were trapped while chipping ice from the Sugar Run air shaft for approx. 1 hour. Ice fell and hit the counter weight buffer switch cable.
The Kuhntown Elevator wen tdown at 7:12 AM with no persons on board. The agencies wer notified at 7:25 M, MSHA and State was on site at this time. The elevator was down until 1:55 PM. The problem was that the ropes needed adjusted and cables re-lubed.
A non-injury event occurred to Camp Run Portal elevator due to the severe weather occurring at the time, 8 miners became trapped on the elevator when power was lost. Back-up electrical power was set, one of the 2 rope grips failed to release. Schindler Elevator service was called to correct the condition. The miners were freed at approximately 12:01 AM 02/06/2010.

Fatal injury due to a rib roll while bolting a belt trench at 240 blk 4 to 7 in 9 south.
EE was uncoupling a mine car from a motor. The jenny couples were connected with a chain. EE had removed the chain and signaled to other motorman to pull off. When pulling the car away from the motor, the EE's right hand was pinched between the chain and jenny couple. EE suffered 3 fractured fingers and amputation to the first distal of the ring finger.
EE was bolting on the alpine miner, both 4' drill steels were stuck in the roof. EE was bringing the 2' steel to meet with the stuck steel. When meeting up with the other steels the 2' steel kicked to the right pinning the EE hand against the steel and bolter mast. EE received 19 sutures and fracture to the index finger.
EE was stepping off the miner, when he stepped on a piece of rock, twisting his right ankle. EE continued to work until 1/4/11 when he could no longer work with the pain.
EE was standing behind BT.22 mobile which was off track in a track spurr. The mobile was struck with a 20 ton motor which caused the mobile to kick sideways pinning the ee feet beneath the mobile and the rib line.
EE injured back of left hand when it came in contact with the rollers while inspecting the Sugar Run Elevator.
Injured was installing roof bolts when his drill steel became lodged and bent as he freed the steel and lowered it from the roof he reached in to grab the still and inadvertently bumped the bolters control lever rotating the steel sticking him on the inside of the right arm and hand causing a fracture to his thumb.
EE was lifting eye of choker onto crane and felt pain in his right shoulder.
Employee stepped off barge to tow boat and slipped on ice on the deck of the tow boat. Employee fell hitting left shoulder. Employee continued to work and did not miss work until 2/8/2011. Employee went off work for left shoulder surgery on 2/8/2011
EE was stepping thru a man door at 5 West #2 block belt stopping. EE stepped on a piece of steel cable causing him to slip. Felt pain in left knee and right hip.
While putting left arm through jacket, his left hand struck a rib plate cutting two knuckles.
EE was watching a track rail being bent with a rail bender the bender was being pressurized when the rail broke striking ee on the right shin. EE suffer a bruised shin bone.
Employee was moving a miner cable when he stepped in a hole, twisting his left knee.
EE was making a hole in a block stopping upper right corner with a tiack hammer, when attempting to strike the wall the hammer hit the metal strap and dislodged a piece of rock 12' dia, ee was struck on the right shoulder. EE complained next day loss of grip and some numbness to the right side of neck.
Two bolter operators were re-bolting an area that was breaking up. They had bolted to a simplex jack. They decided to manually remove the jack. While one bolter started to let the jack down the other bolter pulled back on the mesh. At this time a rock fell out of the roof between the roof bolts striking the bolter pulling the mesh back on the legs causing a fracture.
Injured was kicking with his foot a 8""x8""x3' crib block under a fifth wheel trailer so he could un hook it from a prime mover truck. He missed the crib block with one of his kicks and felt a sharp pain in his left knee falling to the ground and laying there until he received help to get up. Employee started missing work on December 14th 2010
EE was lifting a pump out of a sump when the pump slipped from his hands. EE grabbed for the pump and wrenched the right arm and shoulder, starting at the elbow. EE continued to work until 1/10/11 when his restriction for transitional work could not be met.
EE lost his balance while using a water hose and reached with his left hand for support, grabbing a belt roller smashing his left middle and index finger tips.
EE dropped a water hose to a concert floor of the bunker engaging the valve causing it to flip up and strike his left eye area.
EE was operating the head drum of the long wall shearer a rock was thrown from the shearer drum striking the ee on the chin and mouth. EE received 2 broken teeth and 2 cracked teeth.
EE was cleaning coal spillage under the re-claim belt and was walking along an uneven surface. EE twisted the left knee and complained of soreness to the knee. EE continued to work until 12-1-10 when he went off work.
EE was bolting on the right side of the miner in 8 South. He was moving the drill head to unplug it and he had his right hand on the mast and it pinched his fingers.
Working on approach to 3 South seals, using a sledge hammer to break up a rock. Hit the rock 2 times and felt pain in his right shoulder. It was not determined as lost time until 10/11/2010.
Tramming 276 jeep on trolley into 3B section. Approximately at #2 xc an arc flash came out of the trolley breaker service panel. The employee received a burn to his right leg.
EE was de-lugging a track tie and missed causing his forward motion to overextend his swing causing low back pain.
EE was working in a panel on #304 Jeep in the shop. He put his hand on the seat to stand up and the seat turned over pinching his right ring finger causing a laceration and fracture.
Was struck on the left forearm from a suspened drill steel.
EE was prying a track rail slipped and hit his right shoulder against the rib. EE had surgery on 12-2-10 and was determined as reportable by surgeons report dated 12/27/10.
He was on a ladder recovering chains from vent tubing when he fell off the ladder. He landed on his right leg and sprained his right knee.
EE was shoveling along main north belt at 142 block when lifting shovel filled with rock. EE felt pain in the lower back and right hip area. EE continued to work until 1/8/11 when the transitional work could no longer be met.
The employee was using a track jack to put a car back on track. When lowering the jack manually, the top of the jack free fell pinching his left middle finger causing a fracture.
EE was on his knees cleanig K-4 switch with a hammer, when EE started to stand, he felt pain in the lower right back. EE continued to work until 10/26/10 when he went off work due to back pain.
The injured was helping move a roofbolter tire in the supply hole on 14 Right section. They were trying to lift the tire to get some strips of belt form underneath, when he felt pain in his lower back and radiating up his left side.
Pinched left little finger between a cradle roller and the belt while closing a water valve.
Employee was lifting a cylinder out of the truck without sufficient help. The cylinder was greasy causing it to slip from the employees grip. When the cylinder slipped, it lacerated and smashed the employees right pinky finger.
Roof strap fell from the plank jack on the miner and struck EE on his right elbow.
EE was removing track from the 15-W Section. The track rail was raised with a track jack and crib blocks. EE was removing a rail tie with a pry bar when the jack kicked out and the rail moved towards the EE striking the right ankle and top of the foot.
The employee stated that as he was dragging a ventilation tube, he stepped in a rut made by a shuttle car tire and twisted his left knee. The incident became reportable when the employee was given time off work by his doctor on 8-4-2010.
EE was walking main north belt when a piece of coal/rock fell from the roof, striking EE on the left side of the hard hat. EE experienced stiffness to the neck and pain.
Attempting to knock a pin out of spring board of miner with a drift punch. Believes that a piece of punch broke off and became lodged in his left forearm. This left a puncture wound and surgery was required to remove the piece of metal that required 7 sutures to close wound. Punch did not have a mushroomed head.
The employee stated that as he was crossing the handrail of the face conveyor his foot slipped causing his right knee to twist. He felt pain in his right knee.
The injured was on the longwall and was on shield no. 65, which was spotted about half the way in and when he pulled shield no. 66 in the panline pulled back pinning him against the fire hose box mounted in front of shield no. 65, dislocating his right hip.
Employee was pulling blast hose up onto the scaffolding to blast the bent leg.
While loading a bag of rockdust, the employee felt pain in his back. He worked the next day and began a 2 week vacation period on 6-26. He missed work on 7-12 & 7-13, but did not indicate to management that it was work related. He came out of the mine complaining of back pain on 7-21, scheduled a day off with the mine clerk for 7-22, but began missing work on 7-23.
EE stated that he was is 4B section and had lifted one bag of rock dust up and was putting it on his shoulder when he felt a pain in his right stomach area.
The employee stated that while replacing a scrapper blade, he felt pain in his right shoulder. This incident became reportable when the Safety Department received medical evidence on 12-21-2010 that the employee would need surgery to repair his shoulder.
While on #4 belt line moving a rock out of the walkway he felt a pain in his lower back.
Loader was being pulled forward to prepare to rock dust. As the loader was pulled forward, the boom of the loader struck the employee.
Employee was helping a bolter install cable bolts. The bolt did not go all the way in the hole. The operator was spinning bolt. The EE was 8-10 ft. away. When the bolter released pressure on the bolt, the bolt head flew off of the bolt and hit injured on the top of his left shoulder. Employee suffered a hairline fracture to his left clavicle
EE turned around to walk into motor barn when he stepped onto a trolley nip cable and twisted his left knee. EE felt pain in the back of his left knee, his knee popped.
The employee was taking down a set of double doors. The doors had K-panels on the sides. The employee could not get to the hinges. The employee used a 6-inch solid block and attempted to knock panel out. Finger was between block and panel. Employee suffered a fracture to the end of his finger.
EE was dragging 1.5 water hose when hose became stuck. EE jerked on the hose and felt pain in the right shoulder. EE RTW the next sch. shift but went off work 6/28/10 due to cont. pain.
Walking down the steps in the prep plant when his left foot slipped and he felt a pop in his left foot. He fractured a metatarsal bone in his left foot.
Employee received a flash burn to the face, ear and upper chest area when a hose from the track bolter came in contact with the trolley wire.
Employee stated that he was working on the barge in the process of releasing the tension off of the barge winch. He released the winch and the winch crank handle spun around and hit him in the right forearm.
Employee stated that he was on 8 North track between 3B and 4B and had just finished loading steel props for 4B section and was walking on the clearance side back to his motor when he tripped and twisted his ankle
Employee was using a handheld electric drill motor, drilling holes in steel plates. Drill bit caught in the steel, while drilling. Drill motor twisted in the hands of the employee and twisted right wrist. Injury was accessed and employee was advised to see a doctor for a sprained wrist.
The employee stated that as he was crossing the extendable tubing (baloney skin) attached to the back of the auxillary ventilation fan, he lost his balance and fell backwards onto the baloney skin. The force of the exhausting air caused him to jerk back up into the upright position. He later complained of back pain.
EE was walking around the section power center when he tripped over another EE's feet. EE fell to the ground causing pain to the left leg. EE continued to work the remainder of the shift. Later in the day EE went in for medical attention. He has a fractured tibula to the left leg.
EE was bolting on the left side of the Joy miner and finished the bolting cycle. EE walked to the rear of the miner and with no warning the rib (22' long & 13"" thick) rolled out striking him in the head area and pinned him against the miner. EE suffered a broken collar bone & a bruised lung.
EE was changing a jig seeve on the prep plant 4th floor. He was carrying the jig seeve down 4 steps with another ee when the seeve twisted to the right causing ee to receive a cut between the thumb and index finger on the right hand.
EE felt pain in middle of back while unloading impact beds. First work day missed was 5/3/10.
The employee stated that as he was walking around the section parts cart, he struck his left knee against a protruding piece of pipe that had been welded to the side of the cart.
Employee stated that when walking to 8 South section, he was walking down the wire side where there was a rail laying. He stepped on the rail causing hin to trip bending his right foot in an awkward fashion. This resulted in fractures to 3 metatarsal bones in his right foot.
This is being submitted to abate citation # 8026865 - during a part 50 audit. The employee stated that his left foot slipped from the foot hold on the rib protector. He felt a popping sensation in his back. He sustained a back strain and received physical therapy.
EE felt pain in low back and leg after shoveling in the slope pit area. First day missed was 5/6/10.
Employee stated that while working on 2B 8North longwall, he was attempting to put a zip cart onto the handrail of the longwall to transport rockdust to the tailgate. When putting the cart onto the handrail, he caught the last digit of his left middle finger between the cart and the handrail.
Picking up a piece of plastic pipe. Strain of lowere back area.
While hanging the miner cable in a cable hanger, the employee was struck on the head, neck and shoulders by a section of head coal which came from between the roof straps.
In the process of installing a roof bolt off the miner in 2 South rehab area. When installing a cartridge of resin into the hole, he stepped on the step provided on the rib protector and was putting his other foot on the drill mast and when he shifted his weight he felt a ""pop"" on the back side of his right knee.
He had a drill steel stuck in the mine roof and it started to fall. He attempted to grab the steel and it struck him on the right wrist causing a contustion.
EE was walking on a grating surface along a maintenance part and tripped and fell causing pain to his right knee and right shoulder
EE's had the power center in a scoop bucket and were repostioning the power center.The ram of the scoop got caught on the bumper of the power center. The injured EE held a crib block between the ram and the power center to push it loose. The EE's hand slipped off the top of the block and was caught between the block and the transformer. Laceration-Fracture - finger.
EE tripped on a water hose and landed on his right hand causing a fracture to his right radius.
Employee stated that he was roof bolting from the miner in 4B 8North section when he bent over to look at a hose connection on the ATRS when a piece of stone approx. 10"" x 6"" x 1/2"" fell striking him on the left shoulder. This resulted in a non displaced fracture of the scapula.
EE got off the mantrip and started walking,his back started hurting.
EE was dropping bolter cable that was hung from the roof. Tension from the cable caused it to hit the ee in the head, causing him to stumble and twist his right knee.
The ee was changing 6' water valve, stepped over the eater line and struck his knee on bottom. This 7000-1 is submitted to abate citation #8182156 and as a result of a Conference-Litigation decision. The mine operator reserves the right to expunge the record if a court decision is favorable to the mine operator. This accident has been reported by the contractor MVK
EE fell and fractured L3 vertibrace while chipping on top of an overcast to increase the size of on opening.
EE was cleaning the walkway along the 15-W Longwall stage loader, when a large rock rolled from the top of rib striking EE and pining him beneath the rock. EE sustained fractures to both sides of the pelvis and a cracked scapula on the left side.
Employee stated that while bolting off of the miner in 8 South rehab area, he drilled a hole about 8"" into the roof when the drill steel became plugged. He removed the steel and cleaned it and put the drill steel back into the chuck and started to raise it into the hole when the steel hit the lip of the hole causing the steel to bend hitting his right wrist.
Employee was trying to clean windshield of backhoe. He opened the cab door, placed his left hand on the outside handle by the windshield and placed his left foot on the top step, he tried to swing himself out but lost his balance and fell. His right foot caught between the door frame and fender in the cab. He broke the Tibia and Fibula in his right leg.
EE was walking through a regulator opening losing his balance and falling.
The employee stated that while climbing onto the dozer, his foot slipped and he fell, striking his back against the dozer. The incident did not become reportable until 01/29/2010 when the employee began to miss work.
This is being submitted to abate citation # 8026861 - during a part 50 audit. The employee stated that while walking from the face work area towards the section power center, he slipped and fell. He sustained a closed fracture of the coccyx.
EE fell off of overcast when he lost his balance in a high velocity of air, resulting in rib fractures.
The employee stated that while walking from the miner, he was struck in the backside by the loader cable when it unexpectantly pulled loose from a cable hanger by an exiting shuttle car. The incident became reportable when the employee began missing work on 4-3-2010.
While walking the employee came in contact with a partially exposed rib bolt, causing him to fall against a nearby mantrip. The incident became reportable when the employee began missing work on 1/18/2010.
EE felt pain in low back after sliding a belt cutter under the belt. Did not receive diagnosis for injury until 1/20/10.
Drill steel caught the palm of his left hand forcing it against the drill mast while under lateral force. Diag. by a specialist on 12/9/10.
The EE was helping put a tram motor that had de-railed back onto the track. They were using a lever hoist, air bags, and a lifting jack to hoist the motor back onto rails. The EE was assisting with the jack bar as another EE was jacking with the lift jack. The lever hoist slipped, shifting the weight back to the jack motor, motor landed on EE's right leg. EE able to pull leg out.
The ee was adjusting a chain holding an electric motor for a belt drive, the scoop was elevated 10 to 12 inches off the bottom and when the ram was pushed out the scoop hydraulic ram tube came down catching the injured foot This 7000-1 is submitted to abate citation #8182155 This accident has been reported by the contractor MVK
Roof rock fell while drilling and landed on EE's left foot.
EE was loading metal roof straps from the scoop car to the continuous miner. EE raised the straps to allow them to be pushed into the strap rack when he felt pain and a pop in the left shoulder.
EE was dismounting a mobile when his left knee gave out. At this time his right knee struck the edge of the mobile causing pain to the knee. EE was off on S/A until 4/30/10 when he became a L.TA
EE was shoveling at the 15 W belt pony drive when he twisted and felt pain in the left lower side and pain down the left leg. EE continued to work until 6/7/10 went off work due to this injury.
Employee was stepping through an air lock door when the outby airlock wall partially collapsed causing a pressure increase that caught the ee left leg between the man door and the man door frame resulting in a contusion.
EE was changing a bottom belt roller, one side in the cradle and while attempting to raise the roller out of the bracket it fell out bending the right wrist backwards. Pain to his right wrist.
The injured was roofbolting on 14 right section and was installing his last roofbolt when a piece of rock fell from the top and hit him on the left side of the face and neck, causing an abrasion and laceration that required 12 sutures to his neck.
Employee was using a spad gun to install a curtain. A rock dislodged and struck him on the chin.
EE was tying a cable around some steel beams in 2-N old track entry. When standing up, EE caught his left ear on a bent down roof strap. EE received 7 sutures to the left ear.
This incident became reportable when medical evidence was received on December 28, 2010. The employee had received a self inflicted laceration from a razor knife to the left hand while splicing a belt control cable. The laceration required sutures.
Two employees were changing screens on the lower deck of the coarse refuge shaker at the Prep Plant. While one employee was finishing tightening a hold down wedge, his hammer slipped from his hand striking the second employee in the mouth.
Employee was filling bull gears with gear oil and felt pain in his right shoulder. Employee was treated with physical therapy and missed no work. No problems with PPE, mining machines or methods, training, or compliance issues.
Using a stone bar to guide a rail that was being recovered. Stone bar got caught and twisted in his hand causing a laceration to his right thumb.
Employee was picking up a sand prop when another prop fell over pinching his right thumb between the two props resulting in a fracture/laceration.
EE started to pick up a torque arm and lost his balance falling and injuring his right ring finger.
EE was installing a rib bolt and caught his right thumb between the plate and bolt head.
Employee was removing belt structure in 3B Longwall. When a section of bed rail was removed, the belt stand fell over striking him in the left calf causing a laceration.
EE was attaching the return hose to a shield. EE was on his knees on the pontoon when the knee pad slid off the pontoon and hit a different hose that was not connected and had a burr on it. EE cut his upper left thigh. EE rec. 4 staples to the left thigh.
EE was working on the bunker tripper tail belt. He was pushing a belt roller out of a cradle. Another EE was using a ball pein hammer to strike the roller to help remove it. While pushing on the roller EE was struck on the lt middle finger by the hammer. EE continued to work until 9-13-10 then decided to receive medical attn. He received fracture to lt middle finger.
Employee was flagging traffic when a large rock started down hill into traffic. Employee grabbed for the rock and left ring finger got caught between two rocks.
Injured employee was walking from his truck to the building when he slipped in gravel causing him to twist and injure his lower back. He did not fall. At the time of the initial report he was not planning to treat and continued to work. Today, 08/25/2010 he has continued complaints of pain and sought medical treatment.
2x4 slid down a conveyor belt and struck employee on forehead
The employee was removing a K-wall panel when he struck his right knee against a sharp edge of the K-wall panel resulting in a laceration requiring 5 sutures.
EE was riding in an open mantrip when the loop wire that was hanging down came in contact with his lower lip causing a cut , requiring 5 sutures.
This submittal is to abated citation 8031308. While unloading supplies an employee lost his balance and fell against a stack of roof straps. He was transported to a hospital and given pain medication. He returned for the first 2 hours of an 8 hour shift on his next regularly scheduled shift. He was scheduled off on 5-29, 30 & 5-31. He then resumed his normal schedule.
EE fell while filling rockduster pod and cut his right hand.
EE was installing a rib bolt and rib strap and the volt did not tighten up. The ee grabbed the rib strap and jerked on it attempting to pull the rib bole out of the rib when the loose plate struck the ee left wrist causing a laceration (1 inch length).
A twenty ton motor collided with a mantrip causing an employee to receive a cut on his right bicep.
Employee was trimming back e portion of a cable jacket, when his knife slipped and he cut his left thumb.
The report is being submitted to abate citation 8031318. The employee stepped backwards, tripped and later complained of low back pain. The employee doctor's slips for 5-12, 5-19, 5-26 & 6-18, did not indicate that they were in any way related to this incident.
EE was performing as a rig worker lifting bales of hay. Injury was for a lower back sprain/strain. EE was injured on 4/21/10. Injury reportedly did not occur o DD Company property. EE was off for 29 days in regard to claim #2010007356. This accident occurred at Metz/Rymer while mulching hay for reclaiming site.
EE smashed middle finger on left hand when a track rail fell on the track tie the ee was installing.
EE was cut on the left ear when struck by a jack bar while re-railing a mantrip.
EE came in contact with a roof strap hanging between two shields and cut his left ear. Requiring 10 stitches.
While replacing an ATRS jack, the employee stated that when the jack was freed-up, it rotated forward and the employee's right forearm was caught between the frame and the jack.
EE was changing a bearing on a belt head roller, when he pinched his left index finger while manually rolling a chain sprocket.
EE was testing the fire suppression at a charging station, when he pressurized the system, the spray bar came unhooked and struck him in the cheek. Knocked him into the rib, cutting his forehead on coal, received 5 sutures.
While trying to famillarize himself with functions of levers for fletcher bolter that was located at 8 wouth rehab area. he actlvated drill head twist lever and in doing so drill head caught a drill steel causing it to flip up and strike him in the mouth. broken front tooth.
EE fell while doing a pre-op inspection of a 637 E pan at the fueling station, injuring his right leg. Was diagnosed with a fractured fibula on 2/4/10.
EE was attemptimg to re-rail an Irwin Carrier loaded with a shield by scotching the wheel with a board. The board kicked out catching his left ring finger betwee the board and the frame of the car. EE received sutures to the left ring finger. EE returned to work the next scheduled shift.
The miner was employed at this mine at the time the occupational illness claim was filed/reported. The claim was reported on 11/6/2010 for hearing loss. 7000-1 filled out to abate citation issued on 8/28/2014.
EE was diagnosed with 0.18% of noise induced hearing loss. Loverride was notified on 10/18/10.
The mine received notification on 9-29-2010 that the employee was granted a hearing loss award.
While cleaning an open top mud tank the employee experienced dizziness, slight nausea, muscle spasms, and loss of equilibrium, he was given asprain, water and cold compresses for his head and neck area to cool him down. An ambulance was summoned to take him for treatment at wheeling hospital, he was treated for head exhaustion and released.
The EE was diagnosed with 0.73% permanent partial disability as result of noise. Consol. was notified on 4/19/2010.
EE was awarded 10% occupational pneumoconiosis. Loveridge received notice on Jan 29, 2010.
