Over-exertion in lifting objects

Worker was lifting stone from a conveyor belt to be stacked on a pallet, he felt a sharp pain in his side, which caused an existing hernia to become larger. Worker will be having surgery to repair this hernia on 07/05/2007

Over-exertion in pulling or pushing objects

Worker was repairing a tire in a skid loader when he twisted and pulled on the tire he injured his right arm, tearing the bicept from the bone.

Worker had been working on a saw, he was reasigned to catch stone from a conveyor and stack the stone on pallets. He asked to go back to the saw and was told NO. Soon after his back started to ache, he could not remember an injury. Dr diagnosis: low back pain.

Over-exertion in lifting objects

Worker stated that he was lifting a large stone from a conveyor to a pallet and felt a strain in his back. He did not report the injury until 10/26/2007. He felt that the injury would heal itself.

Worker was lifting a piece of stone he felt a pull in his left groin area resulting in a groin strain.

Over-exertion in lifting objects

Worker was removing stone from a table; stone broke in half, worker tried to catch the stone causing a strain to his right shoulder. Worker was placed on light duty. On 07/30/2007 worker requested to seek medical attention.

Fall to the walkway or working surface

Worker was moving a piece of stone; he tripped and fell feeling pain in his leg and lower back. He did not report his injury to the company until 07/30/2007. He was placed on company-ordered light duty. His pain continued; on 08/20/2007 he was sent to the clnic diagnosed with lumbar strain. meds and therapy. restricted duty.

Worker was spliting stone using a hammer and chisel when he struck his finger with the hammer causing a crushing injury to his finger.

Struck by flying object

Worker was using a hammer and chisel to dress stone when a small piece of steel broke from the hammer and entered his right thumb. Dr removed steel and stitched wound.

Worker was wrapping a pallet of stone with shrink wrap. He pulled hard on the wrap, causing the roll to slip from his hands. The roll struck him in the face and hit his safety glasses. Safety glasses struck right cheek, causing a laceration below the right eye, requiring 4 stiches to close.

Caught in, under or between a moving and a stationary object

Employee was moving a piece of stone when he pinched his left hand between the stone he was moving an another piece of stone. He treated the injury with ice for one week then went to the clinic. Employee has a contusion to the hand; doctor perscribed meds, to reduce swelling. No restricted, no lost time.

Struck by falling object

Worker was moving large pieces of stone, when one fell, hitting the little finger of his right hand, causing a closed fracture to the finger.

Struck against a moving object

Employee reported to me that he has been having pain in his right shoulder for some time; he feels that it is a work-related repetitive motion injury.

Worker was splitting stone using a hammer and chisel. Missed the chisel striking his left hand between #1 finger and thumb on the backside of his hand causing a contusion to the hand.
