Struck by... (Not Elsewhere Classified)

Worker was splitting a stone using a hammer and chisel when a small piece of the chisel broke off and struck him on the inside of his left leg just under his knee. First aid was given for a small puncture wound. Worker was sent to the Dr. on 09/30/2013 and put on restricted duty.

Fall to the walkway or working surface

Worker was lifting and carrying a piece of stone to be stacked on a pallet when he lost his balance and fell on his right side causing a strain. No lost time, reassigned to restricted duty.

Over-exertion in wielding or throwing objects

Worker was splitting stone using a hammer and chisel when he felt a pain in his Left wrist. Reported the injury and was treated with ice. 06/11/2013 reported for work and requested to see a Dr. went to clinic. (sprained wrist) restricted, NO LOST TIME!

Over-exertion in lifting objects

Worker was lifting and stacking stone when he felt a pain in his lower back. He continued to work for the rest of the day (04/15/2013) reporting his injury the following morning 04/16/2013. Went to clinic-lumbar strain-restricted duty and meds.
