Over-exertion in lifting objects

Worker was picking up a piece of stone and felt a pull in his lower back. Went to Dr. and was given meds. and restrictions.

Over-exertion in lifting objects

Worker was stacking stone. Felt a pull/pain on his left side upper rib cage. Went to Dr. = rib strain. Meds & restricted duty. No lost time.

Over-exertion (Not Elsewhere Classified)

Worker was bent over splitting stone using a hammer and chisel, felt pain in lower back and left leg. Sent to clinic on 09/21/2009. No lost time, no restrictions. Meds only.

Over-exertion in lifting objects

Worker was flipping a large piece of stone and strained his lower back.

Caught in, under or between a moving and a stationary object

Operator was cutting stone using a guillitone cutter. When he cut the stone, his L hand was under the back of the stone. The stone fell backwards on his hand, causing a fracture to the middle finger of his L hand. No Lost time.

Struck by flying object

Worker was splitting stone using a hammer and chisel. A small piece of steel broke from the chisel and lodged in his left leg just below the knee.
