Struck by falling object

Worker was stacking stone; the stone slipped and fell on his left hand causing fractures to the tips of fingers #2 & 3.

Worker struck left hand with hammer while spliting stone, using hammer and chisel.

Employee was lifting a large piece of stone and injured his lower back.

Worker was lifting a stone when he felt a pain in his right side, he continued to work but the pain grew worse. Reported to the clinic, diagnosised with groin/hip strain. Meds given/restricted to 20 lbs. lifting.

Caught in, under or between a moving and a stationary object

Worker was moving a stone to be cut. Stone slipped and pinched his left thumb between this stone and another, causing a fracture to the thumb.

Over-exertion (Not Elsewhere Classified)

Worker was bent over splitting stone using a hammer and chisel. He felt a pain in his lower back and left leg. Injury was reported but the worker did not inform the company that he wanted to see a Dr. until 10/01/2008. Worker has a lumbar strain. No restrictions, no lost time. Meds. only.

Worker was carring a piece of stone and he slipped on some ice, he felt a pain in his hip, he tried to self treat his injury with limited results. Went to the clinic on 04/01/2008 and was diagnosised with a groin/hip strain. Worker in not restricted but was given; NSAID

Inhalation of radiations, caustics, toxic and noxious substances

Building on mine property was over-exposed to carbon monoxide gas from an improperly tuned forklift. Worker was taken for medical treatment. No work time was lost.

Worker's right shoulder became sore from 30 years of operating equipment. Chose to do company assigned light duty until 10/27/2008, at which time he went to Dr. and was treated with meds and restrictions.

Worker was exiting a loader, he hopped off the last step and he felt a pain in his abdomen. Went to doctor and was diagnosed with a umbilical herna.
