Employee finished his shift, went home, had dinner, spoke with his family, went to bed. Woke up with back pain sometime on 08/01/2014 and went to the emergency room. Was treated and released to return to work with restrictions.

Worker was operating a forklift. He went to step down from the forklift and felt a pull in his left calf.

Caught in, under or between a moving and a stationary object

Worker was splitting a stone using a hammer and chisel when the stone fractured trapping his gloved hand and the #4 finger of his right hand causing a open fracture of the finger.

Struck against a moving object

Worker was operating a loader removing stone from a ledge. He felt his back tighten and soon it became painful. This injury was NOT reported to the company. The worker went to the doctor and was put on restricted duty.
