Checking the bank, a rock rolled out and hit the employee. He as approx 40' from the bank.
EE was walking out of the guard shack to assist a vehicle when he slipped on some ice and landed on his left shoulder. This weather is unusual for this area. There was also snow on the ground. EE went to the Mt. Graham Hospital to receive treatment, and was released with a bruised shoulder.
Moving new fire hydrant in ditch to obtain stamped numbers for documentation, another employee pushed the hydrant past vertical and it moved to the other side of the ditch. The injured employee went with the hydrant to the other side of the ditch. His foot got caught between the hydrant riser and the ditch, causing a displaced calcaneus fracture resulting in 2 days lost time.
When removing hydraulic filter, strained shoulder.
Climing down ladder felt pain in stomach, had strain in stomach area may be hernia.
While setting a pull of anodes into the staging rack, an anode insulator caught on the rack. The employee attempted to correct the problem by holding on to the anode. The anode fell and struck the employee on his left hand, resulting in a fractured thumb.
Employee was working at the 4000 level putting cable on the arm of a cable truck. The cable caught on the bottom of the arm and fell. The employee's thumb was caught by the loop and bent his hand backwards.
Employees were loading spare anodes back into their shipping crates. One of the anodes hung up on the crate and when freed, it fell and landed on the crate and smashed his right thumb.
Employees were moving anodes on a rail system. One of the employees accidently pushed an anodes off the rails while trying to move it. The anode fell and landed on the employee's foot, fracturing his toe.
Employee was installing new hoist cables on the 1 shovel. Employee was standing inside of the dipper attempting to get rear cable on the bail of the dipper. A cable sprang loose and struck his leg.
Employee was working at the wood salvage yard. While removing a 2x4 from the recycle pile, he felt pain in his back.
Injured was using a hydraulic jack to level trunion bases. The angle iron under the jack slipped off the shims, causing it to fall toward the injured. The injured put his hand out to slow / deflect the angle iron. This caused a fracture to the injured's wrist. No lost days. Restricted duty only
Employee was walking on the leach pad when he twisted his ankle.
Employee was in the rear swing compartment preparing to tac weld a steel plate (33 X 5 X 2). Employee was holding the plate with his left hand. He reached with his right hand for the welding stinger, the steel plate began to wobble and fell out. As the employee tried to move out of the way of the falling plate, it struck his right hand fracturing his second finger in two places.
Employee was assisting a crew installing a bushing on a 2800 shovel. The employee used a hammer to strike the bushing to help drive it in place. When he struck the bearing (two hardened surfaces) a small chip of steel broke off and embedded in his knee.
Employee tested a 4160 Volt transformer with a 600 Volt Meter. Employee was not instructed to test the transfomer, but a panel inside the building. The meter flashed and the employee was subjected to the arc flash.
While working putting in duct work employee caught his fingers between two pipes. Another employee accidentally hit one of the pipes and caused his finger to get caught in a pinch point.
Employee was working below other crews. One of the crew above dropped a tool which bounced and hit injured in the back of the head. Tool hit hard hat first no days away.
Setting impact idler, caught finger between impact idler and structural steel brace. Fracture to right ringfinger. Required sutures
Employee was cutting an electrical cable tray and the Metabo kicked back and hit his hand.
While hooking up a light plant to a pickup truck in the Safford Mine, the equipment operator was assisting his supervisor and pinched his right index finger which resulted in an avulsion.
Injured was inventoring handrails in laydown yard. He was walking backwards and stepped on a railroad tie causing his foot to twist. Employee was taken for further evaluation approximately a week later when there was still pain and discomfort. Upon further evaluation it was determined that some metatarceis had sustained fractures. No lost time or restricted duties.
Employee was placing crosstie under idler bracket. Employee's hand was over the end of the tie when the tie was moved on the opposite end causing his hand to get caught in a pinch point sustaining a fractured thumb. No days away. No restrictions.
Working on 569 truck in engine compartment and became light headed. Came to shop for aspirin and Phelps dodge personel sent him to paramedic and they put ice on his neck and called in the paramedic team. Took him to the ambulance and put the heat montior on him and contacted the doctor. Dr. decided that he needed to go the hospital to be checked out.
