Struck by powered moving object

EE was commuting home on his motorcycle, he was with his job, when he was hit by another contractor on site who was driving a truck; that contractor is responsible for the accident and injury to EE.

While three mechanics were loading a torch box onto a flatbed truck, one mechanic heard a pop in his left arm near the bicep resulting in a ruptured bicep tendon.

When Shovel 1 experienced a fault while loading Haul Truck 17, the dipper lowered into the bed of the haul truck. The haul truck driver was jostled in her seat while waiting to be loaded.

While removing idlers from the 2210 Feeder 002 conveyor in the Secondary Crusher, the mechanic felt pain in his right shoulder.

While lifting an electrical conduit reel into the bed of a pickup truck, the electrician apprentice felt pain in his lower back.

Employee was assisting in doing repairs to the drop chute on the 2140. The employee was positioned inside the chute using an Allen wrench to remove bolts on the liners. The employee positioned outside was using 3/4"" drive impact. Mis-communication resulted in the employee inside the chute positioning his hand over a bolt that was being loosened by the impact wrench.

Caught in, under or between a moving and a stationary object

Employee was unloading rail road ties and caught his finger in a pinch point between the tie and the bed of the flat bed truck.

Struck by flying object

Employee was making adjustments to the trunions on the agglomerator drum. Small particles of graphite were air borne. When employee completed job, he felt something in his right eye.

Contact with hot objects or substances

The employee was replacing liners in the bin. The liner was hot tacked in place when he struck it with a 4-pound hammer to further position it. When the 4-pound hammer contacted the liner at a glancing angle instead of straight on, the hot tack broke and was projected behind the safety glassess into the left eye.

Absorption of radiations, caustics, toxic and noxious substances

Employee was repairing a 3/4"" plug on a 4"" HDPE pipe while on Cell 12 of the Leachpad. Employee was trying to tighten the 3/4"" plug with a pair of channel locks when it sprayed raffinate towards his face.

Struck by falling object

Employee was working under the cathode conveyor when a foreign object fell into his eye.

Fall from machine

Employee was dismounting a Caterpillar 735 articulating dump truck when he fell approximately 6 feet to the ground.

Crew was flying in a piece of 8"" pipe with a 90 degree bend 18' X 18'. An initial pick was made and a decision to re-rig was made. The pipe was placed on a concrete slope 1:1. The pipe stayed in place momentarily and then slid down slope aabout 5 feet catching the injured between the pipe and a crane outrigger.

Employee was assisting another employee with stripping wax from a vinyl tile floor. She was mopping a floor that had just been stripped. She slipped and fell on her behind. She was sent to the hospital where x-rays were taken and diagnosed as a bruised tailbone.

Employee was changing picks on grader. When the employee swung his hammer it glanced off the pick. The employee's thumb contacted a burr on the pick lacerating his hand.

Employee was replacing feeder liners in the 2130 feeder #1. He felt ill at the end of the shift. He was treated for dehydration.
