Caught in, under or between running or meshing objects

Employee was cleaning out vertical auger when he accidentally hit the ON switch while leaving his hand in the clean out area. Amputated Index & Middle Finger of right hand

Struck against a moving object

Employee experienced strain in lower back after the front end of his grader was struck by a haul truck.

Contact with heat

While working at the tankhouse, the EW Operator experienced muscle cramps, dry mouth and was treated for dehydration.

Struck by powered moving object

Employee was struck by a four-wheeler ATV on the Leach Pad.

Struck by falling object

Individual with another security officer were attempting to move a portable stop sign from side of entry way to the middle of the entry way. A gust of wind caught the sign causing it to strike the employee on the neck.

Employee was hauling a load of 8"" I-Beams on a forklift. One of the beams started to fall off the stack and the employee attempted to replace the beam on the stack. It fell off the stack and landed on his left foot above the steel toe.

While hooking up the reel machine trailer to a four-wheeler ATV, the ATV operator backed up and struck a Leaching employee. The employee fell to the ground and the rear right tire ran over the employee's left upper arm and left leg.

Employee was moving a manlift, lost footing while going over rough spot on road, leg slipped through entry and received laceration.

Over-exertion in lifting objects

Employee was lifting a trougher roller onto his left shoulder, when he felt pain.

Trying to extract a bolt from hoist drum with the hammer and bolt, when the hammer slipped off the head of the bolt and hit his little finger.

Fall to the walkway or working surface

The employee was in the restroom at the fuel dock. The floor was wet and muddy. The employee slipped and fell, hitting her tail bone on the floor.

Over-exertion (Not Elsewhere Classified)

Employee was originally injured on 9-8-2008 while tripping out HQ rods.He felt pain in his low back. Employee saw his doctor and was released back to full duty. On 10-1-2008 employee complained that the mine road hurt his back to drive on it. He was taken to the clinic and put on light duty for two weeks for a low back strain.

Employee was working on the 2410 portable conveyor, and was in the process of removing the soft start panel from the electrical cabinet. When he attempted to remove the soft start his hand was pinched between it and the cabinet frame.

Employees were moving a piece of 6"" steel tubing. An employee on the opposite end dropped his side of the tubing, casuing the injured to catch his finger in a pinch point. Medical treatment - No days away

Employee was stepping off of his service truck when he slipped and landed on his elbow.

Employee stepped on a rock and twisted her knee.

Fall onto or against objects

Employee was installing the cover on the dust collector at the Primary Crusher. As the load was being lifted, his left foot slipped and it struck either the suspended load or the structure.

The operator of the HT 21 dropped her flashlight under the right side of the driver's seat. When she reached under the seat to retrieve the flashlight, her right hand came in contact with a cotter pin resulting in a one inch cut to the hand.

Employee was helping replace the liners in the secondary crusher on Saturday 9/13/08 between 8 & 9 pm when he believes something entered his eye. He never thought any mre about it until Sunday morning. He was not welding/grinding or cutting when something entered his eye.

The cathode lifting tool that the employee was using to realign the conveyor chain slipped off the chain and struck the employee on the chin.

The employee saw banding on the ground and picked it up to toss into the dumpster. As he threw it he cut his little finger on his left hand.

While two Crush and Convey maintenance employees were unloading a length of 6 inch by 6 inch angle iron from a flatbed truck, the metal slid off and struck one employee on the lower leg resulting in a puncture wound.
