Accident type, without injuries

The agglomerate discharge conveyor 2350 caught fire. The ignition source was a failed bearing on an impact idler. Confirmed fire at 4:49AM and extinguished fire at 5:33AM

Fall to the walkway or working surface

Pulling wire and ran out of wire. Changed reels, and spliced new wire. When letting out slack the rope started burning hand. let go of rope and the wire hit right leg and knocked the employee down.

Contact with electrical current

Employee was performing routine maintenance on the propel motor on shovel 4 when he experienced electric shock.

Struck against a moving object

Employee stated while being loaded by the #2 Shovel a rock fell off the dipper causing haul truck #24 to bounce. She began to get a headache, reporting the event to her supervisor later in the shift.

Over-exertion (Not Elsewhere Classified)

Employee stated his knee had pain while rushing to get to the man van after shift. This was not reported to this supervisor until the following morning. Employee sought treatment on his own prior to reporting.

Employee was spotting an ATV pulling 4"" HDPE pipe. While disconnecting the pipe from the ATV, the operator of the ATV went in reverse and rolled the tire up the employee's right leg.

Over-exertion in lifting objects

Employee moving rocks by hand and strained his shoulder.

Employee was manually rotating the drive belt on the auxiliary boiler pump, when his left ring finger was pinched between the drive belt and the pulley.

4 man crew was rewinding a belt after prepping ends. 1 man was operating the winder while two helpers insured the belt wound onto the roll uniformly. Injured worker spotted a rock on the belt that he thought would damage the belt when wound onto the roll. He stepped onto the belt to kick the rock, his foot was caught pulling his leg in and fracturing his fibula.

While performing normal job duties, EE experienced normal soreness in his left shoulder that did not go away for a couple of days. Two weeks later while cutting ACSR wire, EE experienced a burning sensation in left shoulder.

Employee was standing on a level section of liner in Zone 7 spotting for D6 dozers spreading material over the liner. When she attempted to turn to her left and walk to another location, she suddenly felt a ""pop"" in the back of her left knee.

Employee was working in the Tail Chute of the 2260 Conveyor. The employee was repositioning himself to remove a liner. In doing so the employee placed his left hand on the wall to climb up, mean while the employee on the outside engaged the impact gun to loosen the nut. The counter sunk bolt spun and lacerated the left palm.

Employee reached with his left arm to help position the cathode strongback and felt pain in left shoulder.

Employee was pulling screen panel with a 1 Ton come along chain hoist. As the screen panel pulled free from its bindings, the employee continued to crank the handle of the hoist. When the screen panel finally pulled free, it did so suddenly, causing the employee to over pull the handle and his elbow made contact with the screen.

Employee was installing a 4"" clamp onto pipe. He then hit the wedge on the clamp with a 4lbs. hammer causing the wedge to break and strike him in the mouth.

Employee suffered chipped tooth while driving T-post with post driver. Post driver caught tooth on T-post striking employee's hard hat and chipping employee's tooth. Employee seen by dentist and repaired with resin and returned to work.

Employee was changing a left side troughing roller near the head of the 2280 CON 001. While reinstalling the left side roller, the employee was supporting the weight with his left hand and trying to start a frame bolt with his right hand. The roller shifted causing the employees finger to be caught between it and the angle iron frame.

Over-exertion in pulling or pushing objects

While Employee was pulling a ground wire, which went from the radio to the converter, the wire popped up and struck the Employee in the left eye.

Employee got out of SEC-07 Pickup and inattentively shut the door with his right hand still in the door. Employee had to open the driver's side door to remove his finger. The finger sustained a laceration which required stitches.

Employee was spotting a forklift unloading drill steel off of a trailer. The load fell off of the forks and one of the drill steel struck employee in the mouth.

Employee was adjusting liners inside feeder chute. While using a 3 lb hammer to raise the liner by hitting the picking eye, the picking eye broke loose and struck the employee in the mouth.

Employee was assisting in changing out the drill bit on Drill 2. Employee was signaling to the operator in the cab and took his eyes off his hands. As the drill bit and basket bottomed out on the drill pad, the basket and bit slammed together.

Employee was attempting to clean window and shut driver's side door on finger.

EE was standing on the crusher deck of the Fine Crushing Plant. It was windy outside and a gust of wind blew causing a piece of dust to go into the EE's left eye. EE was wearing safety glasses when the incident occurred.

EE was using the service crane to left a copper cathode out of the wash tank when the clamp that was connected to the cathode slipped off and struck the EE in the center of the mouth.

Employee was assisting driller to change drill steel bit. The employee placed a small steel plate for a spacer between drill steel and tool wrench. When the wrench closed, the employee pinched left index finger.
