Employee was lifting skids (50-60 lbs.) when he felt pain in his back.
While employee was removing a chute liner, the muddy liner slipped through the employee's gloves and landed on employee's left foot behind the safety toe protection.
Employee was cleaning the Shovel pit using the rubber tire dozer. Employee had made an initial pass and was backing up when she struck a boulder.
Employee alleges that while carrying, dragging, and rolling 20 inch accordion air ducts with two other employees that he felt a sharp burning pain in his lower right back. The employee never reported the incident to his supervisor or other employees. The employee reported the incident on January 17, 2016.
As haul truck operator was preparing to dismount, the cab filter door was open and caught employee's right pant leg causing her to trip and fall onto haul truck deck.
Drill crew was tripping out HQ Rods and as both driller assistants were sliding the 20' section back onto the rod table, one assistant had his right hand between the rod and the pipe on the rod rack, causing him to pinch his right middle finger between rods. The nail was partially removed and x-rays indicated a tuft fracture of the finger tip.
Employee lost his balance while removing guarding to perform are clean up. Employee pinched his right index finger between the structure and the guarding he was holding.
Employee states she had been experiencing skin irritation on the back of her neck and inside of her forearms. Employee states this irritation has occurred intermittently for approximately one month. Employee states skin irritation seems to worsen during shift rotations.
