Struck by falling object

Employee was assisting with loading of Drill Steel onto the bed of the Boom Truck. The forklift operator set the Drill Steel into the cradle and backed away. Employee stepped forward to begi n tie down of Drill Steel when the Drill Steel rolled off and struck the employee in the lower left leg.

Over-exertion in lifting objects

Employee was lifting a bundle of wood stakes to put into the back of the pickup. While doing so, the bundle of wood stakes hit the buggy whip causing the wood stakes to slip away from his hands. Employee tried to catch the wood stakes and twisted his upper body.

Employee states that he finished cutting and welding on the tripper (latch bar) & dutchman on Shovel 4 and shortly after felt discomfort in his left eye.

Contact with electrical current

Employee went to open holding tank drain valve and touched a recently installed light post fixture at the same time and received a minor shock. Employee's first day of lost time was on 7/27/11

Struck by flying object

While a mechanic was hammering a track pin on Shovel 3, a fragment of metal dislodged from the pin and embedded into the upper leg of an employee who was standing approximately 3-4' away.

Fall to the walkway or working surface

Employee crossed a berm on foot and tripped and fell causing a fracture to his ankle.

Over-exertion (Not Elsewhere Classified)

Employee was assisting pushing in 3"" line when he lost his footing and extended left ankle. Supplemental from 3/25/2011: Employee was walking down a shot-created slope in the raffinate area and slipped on debris. Send for x-rays on 3/25/2011 and confirmed fractured fibula.

Two maintenance employees were raising liners on a discharge chute which was on ground level. One employee was positioned inside with a four pound hammer while the other employee was outside with the impact drill. The employee inside began to hammer on the liner, the employee outside noticed that it was stuck and positioned his hands to push when his left index finger was struck.

Struck against stationary object

Employee bumped his knee on an access panel door which is located in front of the Haul Truck cab.

Employee was assisting in freeing the last section of 2"" HDPE pipe that was tangled on the pipe reel. The Employee was positioned in front of the reel. The end of the pipe sprung back and contacted the Employee in the face (top lip area) causing a laceration and dislodged the upper right front tooth.

Caught in, under or between a moving and a stationary object

Mechanic was installing a bogie roller on a spare portable carbody. Mechanic was attempting to position the roller and had the flange of the roller on top of the track rail. The Bogie roller slipped into place pinching his right pinky finger between the roller and the track rail.

Struck by falling object

Employee was removing siding from insulation fab shop for demobe activities, when the building shifted and fell striking employee.

Struck by flying object

Employee was operating the Haul Truck 21 since 06:30pm. At 12:30am he called his supervisor and stated that his left eye was hurting and that he thought it was from the dust. Employee received a scratch to the cornea due to the fact that dust was between the contact lens and his eye.
