Over-exertion (Not Elsewhere Classified)

Employee was climbing the stairs to the cab of a haul truck when he felt a pop in his left knee.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was driving a haul truck when he thought his truck was on fire and discharged the fire suppression system. The employee now claims to have difficulty breathing due to this event and received days away from work during a follow up appointment on 10/8/13.

Fall from machine

Employee was checking engine oil on a rubber tire dozer. As he was dismounting the rubber tire dozer, his foot became caught on a step, causing him to fall approximately 2 feet to the ground.

Caught in, under or between a moving and a stationary object

While moving the J-hook out of the line of fire, the hook was between the upper and lower rails. Employee was trying to relocate the hook. The hook caught a hydraulic line while it was moving in the down position. Causing fingers to get smashed between the railing and the j-hook.

Fall to the walkway or working surface

Dusty concrete slope, employee slipped and lost his footing, broke his right Fibula as a result.

Employee parked the Prinoth service vehicle alongside the Overland corridor. When exiting the vehicle cab, he placed foot on the track and the other onto the ground. During this process he felt a popping sensation in his right knee. The employee stated that he had 3 points of contact.

Employee was raising the electric boarding ladder on a haul truck when her arm was pinched between the ladder and the body of the truck.

Employee was changing drill bit when he pinched his thumb between the drill steel and the lifting bell.

Grinding, accumulated debris on face shield fell into eye.

Employee attempted to dislodge a pry bar from a plugged chute with his right foot. The pry bar gave way resulting in an ankle fracture.

Employee was removing ground cable at the E3 mixer, while he was cutting he tie straps off with a pocket knife he cut his left middle finger.

Employee was draining acid line. The acid was being drained in containment area of an aid tank. While the acid was draining it ran into the sump area of the containment causing the acid to fume.

While employee was removing zip-tie with pocket knife tool, the blade slipped off and cut his left hand.

Employee was shoveling ceramic saddles. He fell and lacerated his knee and proximal shin.

Employee was attempting to discard a hydraulic hose in the dumpster when the hose uncoiled causing a laceration on the employee's right little finger.

Employee claims that his knee injury is a result of continuous operation of a 994 loader. Employee received work restrictions during a follow up appointment on 08/23/2013.

While cleaning SX filter, elecrolyte solution went into employee's eyes. Employee received medical treatment on 2-28-13 during follow-up medical evauluation.

Struck against stationary object

An employee was washing glassware after running samples. He was washing the Erlenmeyer flask with a brush and the top rim of the flask broke. The broken glassware then cut into the employees left thumb.

Employee was changing the cutting edges on Motor grader #4, When one of the bolts wouldn't come out, the employee used a punch and hammer to remove the bolt. The employee struck his right index finger with the hammer resulting in a laceration that required two stitches.

Employee was assisting the change out of main acid pump in the acid plant. Employee was working in hot weather conditions and began exhibiting signs of heat exhaustion. Headaches and cramping.

Employee was working in a carbody of a shovel when he became overheated.

Employee claims to have hyper extended his right knee while stepping onto a berm. Employee received days away from work on 7-22-13 during a follow up appointment.
