Struck against a moving object

Employee was operating a Haul Truck and was in position, getting loaded by Shovel 1. Shovel 1 lost power and the dipper fell into bed of Haul Truck. Incident did not become a reportable incident until 8/6/2010 when employee was placed on restricted duty.

Keeper came free and track section came down on LT foot pinning it down.

While removing asphalt on the Freeport-McMoRan Road, the employee placed his left hand on a large piece of asphalt, while another employee tossed a piece of asphalt that struck his left hand.

Fall to the walkway or working surface

Employee was leaving the building after line-up tailgate meeting. As she exited the building, she stepped on a small rock, twisting her left ankle, causing her to fall down and scrape her right knee. Employee was able to perform normal work duties until 4/26/2010 when she had surgery on her right knee. Employee's incident did not become a lost-time incident until 4/27/2010.

Over-exertion in lifting objects

Employee pulled muscle while lifting railroad ties while working on getting the Radial Stacker un-stuck. The railroad ties were being used for cribbing. The railroad ties had to be lifted by hand and moved by hand to some point. Employee did not feel pain until later.

Struck by falling object

While changing rear access panels on Shovel 2, a panel fell and struck the diagnostic mechanic on the right arm

Employee was driving 12"" rebar stake to support electrical conduit for concrete pour, when 10 lb.sledge hammer slipped off rebar coming in contact with left thumb, causing a laceration and a tuft fracture.

Haul truck driver was walking down steps, when he misstepped and strained his lower back and/or leg.

Struck by flying object

Employee got dust in his eye from another employee grinding behind him. Injured employee was wearing his welding hood and safety glasses. Dust entered from the rear.

Employee was cutting a piece of rubber belting for the tail chute of the conveyor. The belting was placed on the ground on top of some 2 x 4's. The employee was on his knees cutting towards him. The knife blade cut through the belting and punctured the employee's leg.

Caught in, under or between a moving and a stationary object

While removing shelving from storage unit, employee's right ring finger was smashed between shelf being removed and the front support brace

Employee was assisting with cutting strips of conveyor belt. He knelt down and positioned himself such that the utility knife would be cutting towards him. The utility knife hung up on the belting as he was cutting and so he put more force to dislodge the knife and as he did it broke free and the employee stabbed himself in the leg.

Struck against stationary object

Mine mechanic cut his left palm on a metal burr when he was working on the rubber tire dozer.

Contact with heat

Employee was observing work being done on the Leachpad when she began to experience heat-related symptoms.
