Fall to the walkway or working surface

Employee slipped on a rock and fell, scraping left knee.

Over-exertion in pulling or pushing objects

Employee was moving the splice platens on the 2450 radial stacker and felt pain in right elbow.

Struck against a moving object

Employee pre-spotted Haul Truck #29 at Shovel 3. When the operator of Shovel 3 went to clean up material in the Shovel pit, the dipper caught the dove tail, raising the rear of the truck.

Employee stated pain and numbness in left leg when sitting in the smoking area at the Fuel Dock. Employee did not report any prior incident to supervisor.

Employee claims back pain after the first shift riding along in a haul truck. Employee stated it may be due to a prior personal injury. No report of this incident had been made to supervision prior to receipt of workers compensation report from hospital 8/28/18.

Employee was driving Haul Truck 28 when rough loaded by shovel 5. EE felt pain in left shoulder and neck after loading.

Fall down stairs

Employee was carrying tools down a flight of stairs. The employee states ankle gave out and employee fell. This resulted in a fracture to left ankle.

Employee was kneeling down fixing wheels on a welder. Employee stood up and turned to walk away and felt a pop in the lower back.

Employee stepped down approximately 14 inches from a trailer when left knee buckled.

An employee lost control of a 4lb hammer. Striking the injured individual in the left knee. The individual experienced tenderness in the left knee resulting in restrictions to work duties.

EE was changing a tire when ee lifted the tire to install and pulled groin.

Employee was moving 55 gallon drums when they pinched their left middle finger between 2 drums, fracturing the finger tip.

Employee was working with another employee to free the shovel man lift from the catwalk handrail when left thumb was caught in-between the handrail and man lift on Shovel 5.

Employee was passenger in Haul Truck 28 when rough loaded by Shovel 5. EE felt pain in back after loading.

Haul Truck driver was walking down haul truck steps when EE felt EE's left knee pop. EE then felt pain in EE's left knee.

While employee was shoveling loose material, employee twisted and felt pain in lower back.

Employee was shaken in haul truck and felt pain in the back.

Struck by falling object

Two employees were moving sand line sheave to correct location. EE was pushing on the pin with fingers, while co-worker was pulling on the pin. The pin moved suddenly causing EE's fingers to follow the pin into the sheave. The sheave dropped damaging EE's right middle finger and fracturing the ring finger. Three stitches to the ring finger.

Employee had left ring finger pinched while prepping swivel for king pin on radial stacker.

Employee was sitting in the spare seat of a haul truck training another employee when right eye became itchy, irritated then started to blur.

Employee was drilling a hole in pipe when the drill bit hung up causing employee's hand to twist.

Employee was shoveling gravel and felt slight pull in right shoulder.

Employee was using a hammer and drift pin to drive inserts out of a crusher head nut. Employee missed the pin and struck the back of EE's right hand. EE required five sutures to close laceration.

Field Helper was getting EE's equipment ready for the shift. EE was attempting to cut a pulling strap with EE's knife. EE's knife went through the strap too quickly and made contact with EE's hand. The injury required medical attention and stitches to close the wound. EE was cleared to return to work with no restrictions.

Employee was raking material when employee started to feel dizzy and fell down.

Observing work on new liner job near storm water pond.

Employee reported feeling tired and hot after working 3-4 hours into shift. After a 20 minute break in an air conditioned service truck employee reported experiencing muscle cramps in lower jaw.
