Caught in, under or between a moving and a stationary object

Injury involved an experienced worker at jobsite involved in a common/repetitive rigging task for deployment of Geosynthetic Liner materials. EE attempting to insert End Cone into roll inner core, jammed left gloved index finger between tip of cone & inner roll core causing EE to pull hand away which severed the tip of finger above first knuckle. Self inflicted.

Employee was holding the door with a foot while entering the water truck cab. They moved their foot and the wind closed the door on the left middle finger, amputating the fingertip.

Upon exiting the lube truck, employee stepped in loose gravel causing EE to twist EE's right ankle.

Haul truck operator experienced neck pain from a possible loading event at the beginning of the shift and reported neck pain to supervision at the end of the shift. Shovel operator states a rock rolled out of the shovel dipper striking the truck bed. Haul truck operator states feeling a warm sensation in the side of the neck, and some stiffness by the end of the shift.

Employee was removing pre-strips from stainless steel blanks with 3-foot pry bar utilizing a bat swinging motion when right pinky finger came in contact with a pre-stripped piece of copper.

Employee was extracting singles from 4"" pipe using an 18-volt cordless drill and spear. While operating drill employee's hand got wrapped around extracting spear that was attached to drill causing the employee's hand to twist resulting in injured fingers.

Struck by flying object

Mechanics were using a steel hammer to remove old teeth from a 994 loader bucket. A piece of fly metal came off the hammer and entered the employee's right forearm.

Employee complained of sore back after lowering the bed of the haul truck with material in it.

Employee was walking down the stairs when EE felt a pain in the right knee. EE immediately shifted the weight off of knee to relieve pressure to get to the bottom of stairs.

Haul Truck Driver was driving to Shovel 6 at Lone Star pit when it struck a knoll going 6 to 7 mph. The driver felt pain in EE's lower back and numbness in EE's right leg.

Driver of Haul Truck 43 was backing into the shovel 5 pit to be loaded, the driver continued backing into the shovel dipper as the load was dropped into the truck bed, jarring the driver. An hour after the load, the driver noticed a headache and some neck stiffness.

The driver of Haul Truck 28 was being loaded at Shovel 2 leaning fully on EE's right arm on center console, feet not touching the floor boards. The shovel's second dipper loaded caused the operator to feel pain in the right shoulder due to the improper positioning for the task.

While employee was sitting in the passenger seat of a 994 loader, EE heard a pop in the neck while the loader was reversing.

While employee was exiting the man van, the employee rolled an ankle on the running board that had mud build up.

Rubber Tire Dozer 3 was cleaning up shovel 1 when EE hit a bump in the pit which resulted in the operator getting jolted around in the cab causing injury to EE's lower back.

The 1st tooth on the loader bucket made contact with the rear side of the canopy of the haul truck. The truck operator felt a jolt, and experienced lower back pain. The employee received treatment and was cleared to return to work by occupational medicine. On the employee's first day back at work, employee experienced additional pain and was given restrictions.

While stepping out of a pickup truck, the employee stepped on a rock and felt a sharp pain in the middle sole and heel of the right foot.

The employee ran over a rock with a rubber tire dozer while cleaning up a shovel pit, causing a twinge in EE's back. The employee continued to operate the equipment and experienced pain in EE's back and neck after driving over a pothole.

Employee was walking on the leach pad when they stepped in a soft spot causing soreness in their left ankle.

Employee was shoveling ore material and felt a pinch near the middle of back.

EMPLOYEE WAS TOPPING OFF COOLANT ON A 793C AND RAN OUT OF COOLANT. WENT TO CHANGE PUMP FROM EMPTY TOTE TO FULL TOTE. WHILE REMOVING PUMP, EMPLOYEE HIT THEIR RIGHT KNUCKLE ON MACHINE TOW HITCH CUTTING THE FINGER OPEN.

Employee was walking across the parking lot from personal vehicle at the start of shift and felt a pop in left calf muscle.

Employee was opening the truck door when the wind blew it open. It pulled their right arm forward and they felt pain in the right shoulder.

Employee was using a squeegee to pull mud and electrolyte solution into a vac truck pipe and some solution splashed them in the right eye.

Employee was stopping a liner bolt from spinning with a pry bar when it slipped causing ee to cut finger on the spinning bolt.

At the time of the complaint, the employee identified multiple potential mechanisms for the claim. including those of a personal nature. The employee's current description of events includes an onset of pain from repeatedly pulling a lever to dump out the haul truck.

The employee got dust in the eye due to wind, this caused a scratch on the eye, which was treated with prescription medication for greater than one use.

Employee reported a gradual onset of pain in right elbow due to repetitious climbing and movement of elbow.
