THERE WAS A PROBLEM WITH AN ELECTRICAL SWITCH TOTHE BREAK SYSTEM. ALL OTHER HOISTS WERE OPERATIONAL. THE BREAK SWITCH WAS CLEANED AND ELECTRICITY RESTORED.
THERE WAS A POWER FAILURE @ POTASH JUNCTION THAT CAUSED POWER FAILURE AT K-MAG & GRANULATION SURFACE OPERATION AS WELL AS POWER FAILURE TO #2 HOIST. OTHER HOISTS WERE OPERATIONAL.
LIGHTENING STRUCK POWER POLE AND DAMAGED CROSS ARM. ELECTRIC COMPANY CONTRACTOR CAME AND REPLACED BROKEN CROSS BAR. DURING THE DOWNTIME OF HOIST #5, BOTH HOIST #2 AND #4 WERE OPERABLE.
#5 HOIST REPORTED THE BRAKES NOT SETTING EVERY TIME. SHIFT ELECTRICIAN REPORTED THAT THE BRAKE ACCUMULATOR WAS NOT READING THE PRIMARY SWITCH WHEN THE BRAKES WERE RELEASED. WHEN CHECKING AND W ORKING ON BRAKE LINKAGE, A PENCIL WAS FOUND UNDERNEATH THE BRAKE LINKAGE IN THE CONTROL CONSOLE.AFTER THE PENCIL WAS REMOVED, THE LINKAGE ADJUSTED AND BRAKE TESTED.
#1 HOIST (PRODUCTION) DOWN DUE TO PLUGGED FILTER ON THE HYD. ACCUMULATOR ON THE BREAK SYSTEM THAT WOULD NOT RELEASE THE BREAKS, ELECTRICAL LINE STARTER FOR ONE OF THE HYD. PUMPS WOULD NOT COME IN. REPAIRS MADE. ALL SAFETYS CHECKED PRIOR TO STARTUP. 3 OTHER HOISTS AVAILABLE TO HOIST PERSONNEL OUT.
UNPLANNED ROOF FALL OF THE INTERSECTION ONTO THEPANEL COVEYOR BELT. MEASURED 50'L X 45'W X 20"H.NO EMPLOYEE INVOLVED OR INJURED. REMOVAL AND REBOLTING PLANNED.
#5 HOIST ELECTRICAL OUTAGE DUT TO AN ELECTRICAL STORM IN THE IMMEDIATE AREA, ELECTRICIAN FROM PLANT HAD TO RESET BREAKER MANUALLY. UG MINERS HAD 2 OTHER HOISTS TO EGRESS OUT OF THE MINE. VERBA LLY REPORTED TO MSHA ANWERING MACHINE ON 7-30-01
DC LOOP CONTRACTOR WAS NOT PULLING IN HARD ENOUGH TO CLOSE AUXILLIARY CONTACTS @ #5 SHAFT. VERBALLY REPORTED TO MSHA.
THE POWER CABLE FROM THE SUBSTATION TRANSFORMER TO THE SWITCH GEAR GROUNDED OUT ON #5 HOIST. #2 & #4 HOIST WERE OPERATIONAL.
SHIFT MECHANIC WAS ON THE FIRST STEP GOING INTO THE REFINERY OFFICE & NOTICED SUPERVISOR WAS BUSY. AS HE TURNED AROUND TO BACK TOWARDS SHOP, EMPLOYEE FELL AND STRUCK HIS RIGHT KNEE ON THE CONC RETE FLOOR.
EE WAS IN THE PROCESS OF RECOVERING FLOOR MOUNTED BELT STRUCTURE ONT HE 700 LEVEL NEAR AREA 23. THE FLOOR STANDS WERE PARTICALLY BURN IN THE MUCK SO HE AND CO WORKERS WERE PUSHING AND PULLING ONE EACH STAND TO LOOSEN THEM SO THEY COULD BE PICKED UP. WHEN EE PULLED ON THE STAND HE FELT PAIN IN HIS RIGHT SHOULDER.
EMPLOYEE WAS WALKING AROUND THE EAST SIDE OF THE EAST ? SCREEN. EMPLOYEE STEPPED ON SOME WET SLIPPERY PRODUCT(MAG) AND TWISTED HIS LEFT KNEE ANDFELT A POP. EMPLOYEE PLACED ON MODIFIED DUTY.
EE WAS USING AN ARC TO REMOVE STEEL FRO AN OLD WELDING SKID TO REBUILD SKID FOR ANOTHER USE. HE HAD REMOVED A PIECE OF 8" DIAMETER PIPE 19" LONG. AS HE STOOD IT UP ON END AND CONTINUED TO CUT ANOTHER PIECE, THE WELDER LEAD WAS BEHIND THE PIPE AND KNOCKED THE PIECE OF PIPE OVER. IT FELL &STRUCK VICTIM'S LEFT FOOT BEHIND THE STEEL TOE OF HIS BOOT CAUSING A FRACTURE TO HIS BIG TOE.
EES WERE USING A MAGNETIC DRILL THAT WAS POSITIONED ABOUT CHEST HIGH TO THE EE THAT GOT INJURED. THE DRILL BIT SEIZED & THE MAGNETIC DRILL BAR CAME LOOSE & TURNED ONE FULL REVOLUTION BEFORE CO MING TO A STOP. AS THE DRIL BAS SWUNG AROUND IT CAUGHT THE INJURED EE'S L. HAND BETWEEN THE DRILL BASE & THE CORNER OF THE PIECE THAT WAS BEING DRILLED. THIS RESULTED IN A CRUSH INJURY TO EE'
WHILE OPERATING THE MINER IN AREA 8, EE STATES HE WAS USING THE CABLE HOOK TO MOVE THE MINER CABLE TOWARD HIM USING HIS LEFT ARM. HE STATED HE FELT A PULL IN HIS LOWER BACK.
EE RIGGED A CHUCKER THRU ONE OF THE HOLES INT HE BACKUP RING ON THE FLANGE OF THE PIPE. HE SIGNUTED THE GRADELL OPERATOR TO PULLT HE PIPE OUT OF THE WAY TO BE ABLE TO GRAB THE INSTRUCMENT EE G RABBED INSTRUCTMENT T WITH HIS R HAND AND SIGNALTHE OPERATOR TO PULL THE PIPE ALL THE WAY OUT. WHEN THE PIPE CHEARED THE BOTTOM. FLANGE, IT ALLOWED THE PIPE TO TELT OVER. WHEN THE PIPE KILLED
EE WAS PULLING UP THE LO LO WITH THE MINER IN AREA 2. EE GOT OUT TOP OF THE BELT TO SEE IF THE TFACE FAN WAS OUT OF THE TRAVEL WAY BY THE MINER. EE HEARD THE PANEL BELT RIP. THE BELT PULLED IN TO AND FLIPPED THE EE. WHEN EE LANDED HE LANDED ON THE BOTTOM BELT THAT WAS MOVING. BELT PUSHED EE INTO THE CROSS BARS.
EE STATED HE LIFTED A TWO PIECE METAL PLATE GUARD COVER ON #693 DC SHUTTLE TO SEE HOW MUCH CABLE WAS LEFT IN CABLE REEL.WHILE LIFTING THE LEFT GUARD COVER TO UPRIGHT POSITION AND WHILE ATTEMPT ING TO LIFT THE OTHER GUARD COVER THE COVER ON LEFT SIDE FELL. EE GOT HIS LEFT HAND CAUGHT BETWEEN COVER DOOR AND SHUTTLE CAR RESULTING IN LACERATION THAT REQUIRED SUTURES.
EE HAD BEEN TIGHTENING BOLTS ON GEAR CASE AND LOADING OXYGEN TANK ONTO MANTRIP IN AREA 5. EE FELT PAIN IN HIS LOWER BACK.
EE WAS PULLING THE CALL BELL TO RELEASE THE SOUGHT CAGE AT #5 SHAFT AND HIS HAND STRUCK GREAST VENT THAT WAS INSTALLED IN THE GATE HINGE. THIS RESULTED IN EE RECEIVING A SMALL LACERATION TO BA CK OF RT. HAND THAT REQUIRED STITCHES. EE ABLE TO RETURN TO NORMAL DUTIES.
