Over-exertion in lifting objects

Emp was lifting an 8' ladder onto shoulder & back. As emp swung the ladder into place emp felt a pain radiating from left elbow and along forearm. Emp. had Cortisone injection to the left elbow on 9.24.20. Emp. on the following restrictions: light duty, sedentary work. No lost time.

Struck by falling object

An emp. was using a scaling bar to remove rock cores left in the back by the continuous miner. The employee was unable to knock down the core and walked underneath it, the core fell and struck the the emp on the hard hat. Emp. taken to ER for a concussion and given prescription meds in the ER and prescription to take home.

Struck by flying object

Emp installing a water pump. The flex-hose line blew off a coupling between the pipeline hose and pump hose striking emp on the left side, front of head and knocking emp into a chain-link fence. Emp went to ER on 2/23/20 and given 2 prescriptions meds for a headache. CT scan ordered with negative results. Released back to work without restrictions.

Fall down stairs

Going down stairs, slipped and fell forward, hit chest on a concrete foundation. Complained of chest pain and shortness of breath. Taken to ER and given a prescription for pain and released without restrictions.
