Caught in, under or between a moving and a stationary object

EE was removing a roller on UG belt conveyor, right hand was on top of the structure rail. As EE was pushing the roller with EE's left hand, the roller slipped & rolled onto EE's little finger of EE's right hand, pinching/cutting the tip of the finger. EE wearing gloves at the time of injury. EE taken to clinic & received 4 stitches, antibiotic, pain med & placed on restrictions.

Struck by falling object

Employee pulled up to the work zone in forklift to help cherry picker remove material from Level 1. Employee was helping with the outrigger set, heard a clank and someone yell to run. Guard rail was knocked loose by the picker and fell, striking shoulder and forearm.

Struck against a moving object

Emp #1 was operating a ramcar & was parked behind a miner. Ramcar lights were off while ore was being loaded. Emp. #2 was driving another ramcar in reverse, approaching the miner with trailer lights off. Emp #2 didn't see Emp #1 being loaded behind the miner & bumped Emp #1 ramcar. Emp taken to Er per request. Prescription given for pain. Emp. back to work with no restrictions.
