Crack in drum found during routine inspection on #2 hoist. #2 hoist down for investigation & repair. Two other access to mine.

0745 am on #4 shaft down, had intermittent problems with motor generator. 0945 problem solved and #4 shaft back up.

Inspection of # 7 Hoist, showed a low voltage indication. Electrical inspection concluded that a lightning strike had taken out a potential transformer that was tied into the MG power to the # 7 hoist. 3 of our other hoists were operational.

Ore Skip load gate failed to close after dumping ore, skip traveled down and filled with ore, gate failed to close and skip hung in shaft. Miners had access to mine at # 4, # 5 and # 7 shafts.

Struck against stationary object

Employee was pulling on a wrench to loosen center strap screen bolt. The wrench slipped off of the bolt head and the employee hit his elbow and upper arm on the framework of the screen body. Swelling and bruising noted with pain. Evaluated by company doctor on 6/20/07 x-ray negative, place on restrictions. returned to work on restrictive duty on 6/21/07.

Employee stated that he was traveling down the belt on a belt tractor, just made a left turn around the tail piece when he ran into hanging belt structure causing a cut to L upper arm and bruising to L forearm. Emp. to ER received 100 staples to L arm wound (surgical procedure) and a drain. Spent one night in hospital and placed on restricted duty as of 6-15-07.

Original date of injury is 6/11/07. Employee received treatment at the time but is now experiencing increased pain and had a knee replacement on 11/7/07 and has not returned to work at this time. Left knee injury.

Emp complained of pain at wrist on 12/19/07 while removing a drain plug from a gear box with chanel locks (Allen Wrench is the correct tool to use). He didn't feel like he needed to see a doctor so no treatment was done. on 2/21/08 emp went to company doctor due to continuous pain - he was placed on restrictions.

While putting up jumper cables on CU200 picker, employee got the tip of his left index finger caught in the cooling fan. Received laceration to finger and received stitches. On restricted duty.

Emp was shooting the damp scale that had built up in the bottom of the cleanup elevator chute. The opened door came down on his right hand and cut the tip of his right index finger. Taken to ER and then to Orthopedics. Placed on restrictive duty.

employee turned is ankle, went to ER for evaluation. XRAY negative for fracture, placed on restrictions for 5 working days -- 9/30/07 to 10/5/07

Caught in, under or between a moving and a stationary object

Emp. was repairing a broken conveyor chain on a miner. Half of the chain was on the miner & the back half was on the ground. The employee put the chain back on the miner and was pulling it together. The bar slipped and wedged his hand between the chain and the plate causing a laceration. Went to company doctor received sutures & xray positive for fracture. Restrictive duty.

Employee slipped and fell. When falling he put his left hand out to break the fall and the fingers went through the grating. Went to company doctor, placed on restrictions, XRAY negative for fracture.

Employee was changing out bits on miner with a hammer and mashed his left pinky finger between the hammer and the bit wrench causing a fracture and laceration to the left pinky finger. Taken to ER for treatment

Emp. hit right lower leg on the tounge of a trailer that was hooked up to the back of a rig caused bruising and swelling. Went to company doctor 10 days later due to increase swelling and pain. XRAY neg for fracture. Placed on restrictions.

employee was reaching in tool box for a tool and the lid came down and hit him on his right wrist. The next morning the area became swollen and tender, emp taken company doctor. XRay negative, placed on restrictive duty. will re-evaluated on 8/24/07

Contact with hot objects or substances

Employee received 1st and 2nd degree burn to left hand and forearm from hot water and steam while removing a plug on the back of the water strainer on a miner. Went to ER and placed on restrictive duty, pain meds given, and anti inflamatories. Sees company doctor on 8/7/07.

Over-exertion in lifting objects

Employee was picking up a loop of cable and turned to place it on the tail hook when he felt a sharp pain at his lower back. Went to ER on 7/23/07 night, placed on restrictions and will miss two working days.

Employee got out of the ram car and was turning around to face the steps and felt pain in his right knee. Went to company doctor on 7/6/07 and was placed on restrictive duty. Employee was on days off until 7/11/07. He returned to doctor on 7/10/07 and was released to return to work on restrictive duty. Employee is able to do all job duties while following ordered restrictions

Caught in, under or between a moving and a stationary object

Employee was removing a broken bit from the miner. The hammer hit a bit block in front of the bit wrench and deflected, causing him to smash his left ring finger between the hammer and a bit block. Employee to ER received 4 sutures, fracture of tip of left ring finger, immobilation. Saw company doctor on 6/29/07. Returned back to work on restrictive duty on 6/29/07.

Over-exertion in pulling or pushing objects

Employee was pulling up an empty sample bucket and felt pain at his right shoulder. 6/27/07 Employee went to company doctor and a MRI was ordered for 7/5/07. Emp returned to work on restictive duty on 6/28/07.

Fall to the walkway or working surface

Employee had just finished making a splice at belt when he had to go to the back side of the belt to push the by pass button to turn belt on. He rushed back to the belt to spot the next splice and he tripped over the grip hoist rope causing him to fall on his arms and knees. Went to company doctor on 6/22/07 placed on restrictions. Negative x-ray.

4/4/07 Employee stepped off of the top of the skip and felt pain in his right ankle. Slight swelling noted - ice applied. On 6/7/07 employee back to company doctor due to swelling and pain, ordered restrictions that state: no climbing stairs, off of feet every 15 minutes out of every hour. Physical therapy ordered 3 times a week for 2 weeks.

Employee was walking through a doorway and hit his head on a low hanging beam, jamming his neck.

Employee stated he was running a bolter, he had finished installing a bolt and was tramming forward to postion the bolter to another bolt, when he felt a sharp pain at his upper left side of his back.

On 7/12/07 employee was walking up a slope underground while carrying a 5 gal bucket of diesel. He hit his head on the back (with hardhat on) & it caused his neck to jar. Refused tx at that time. On 5/30/08 he requested to see a doctor due to continuing & increased pain in his neck & head. Went to doctor on 6/2/08 received prescription & massage therapy. No restrictions.

Employee loosened troughing rollers on blet line and was taking them off, while others were loading the sled with rollers. One of the rollers behind him fell off and hit him on the right calf and caused a laceration. Employee taken to company doctor and received stitches. Employee was released to return to work at full duty on 5-14-07.

Employee reported that his left eye was irritated after welding and grinding. He drove himself to the doctor(emergency room). A small metal splinter was found and removed from his left eye. He returned to work without restrictions.
