While checking safety checks on # 5 Hoist, hoist would not reset, troubleshooting revealed contacts on Lilly controler were not making up.

Direct short in wiring to the overtravel switch in the headframe of # 5 hoist caused the safety circuit to shut down the hoist. Repaired at 5:20 PM

# 5 Hoist Electrical SCR showed Excessive Current Ripples. SCR was repalced. Mine Employees accessed the mine at # 4 Shaft. Other shafts available for access were # 2 and # 7 shafts. # 5 Hoist back up at 3:00 AM

#5 Hoist outage June 17th from 7:05 am to 7:45 am. Fuses blown on the hoist power supply due to a voltage spike from the utilities supplier incoming power. fuses replace and hoist reset. 3 other hoists available to hoist underground miners out of the mine.

Employee fell through grating cutting his L Leg.

Struck by falling object

NOTE: Unaware this had to be reported, as accident occurred in parking lot outside normal work activities and shift. Emp was parking motorcycle in company parking lot when the motorcycle fell on her, causing fracture to right knee which required surgical repair.

Struck against stationary object

Employee was driving in a low height area and bumped his head against the back. Initial X-ray was negative. Follow up CAT scan was negative. Doctor gave a prescription for muscle relaxant and prescribed one day off work.

Employee stated he had moved 3/4 wire rope (approx 100 ft long) from the vent tube on a belt gear case. The cable jumped, pulling on his left arm and shoulder causing a sharp pain in his shoulder.

Emp had parked ramcar by the belt feeder to help with some cleanup at the end of his shift. He backed the car up and started down BT 41 toward the shop. When he reached down to turn on the lights, he stated that he hit the corner of the rib. The car was not traveling fast and there was no damage to the ramcar.

Over-exertion in lifting objects

Employee reported pain running down his leg when he put on his miner's belt and lamp. Diagnosis was muscle spasm and pressure on sciatic nerve from improper lifting in the recent past.

When employee picked up a bundle of roof bolts, the banding strap caught on another bundle. He felt pain in his left shoulder. When the pain did not go away after three days, he requested to see a doctor. Restricted duty was prescribed.

While operating a conveyor belt stapler, employee felt numbness and tingling in his hands. Restricted duty and further tests were prescribed.

Employee was involved in an altercation on the cage while entering the mine. He attempted to pick up another employee by the pants and pulled a muscle in his right shoulder. Restricted duty and two weeks of physical therapy were prescribed.

The employee engaged in heavy lifting on the day of the injury. He experienced lower back pain the next day. An MRI indicated the possible effects of improper lifting. Restricted duty and 4 weeks physical therapy were prescribed.

Fall to the walkway or working surface

Employee was pulling on a rope to move the arm of a pivot hoist. The rope broke and he fell backward landing on his left hand and wrist. The fall caused compression and simple fractures of the wrist.

Employee moving a 55 gallon drum when he felt pulling and pain in his left shoulder.

Employee stated that he stepped up on a 2' high berm on the road. He felt a sharp pain in his right calf. He was treated with ice. On 8/14/2006 employee notified safety that there was bruising and swelling in right leg. Employee was taken to physican where restrictions and physical therapy was ordered.

Employee missed a step and landed on his right foot causing it to twist; felt sharp pain.

Orignial date of injury is 7/28/06 (first aid incident). He reported on 5/11/07 that he continueds to have pain at the right wrist. He went into company dr and was placed on restrictive duty and given a splint wear on the right wrist while at work. No actual recent injury-just reports of pain.

Emp was installing pipe where the roadway bridge crosses the trench. Emp was pulling on wrench when he felt a pop and pain in his right shoulder.

While stepping on a pile of loose muck, employee felt a pop in his knee. He recently had surgery on that knee. Employee was taken to the company doctor and work restrictions were assigned pending review of an MRI.

Injured person had just installed a chain fall on a beam. The beam clamp slipped and the chainfall struck his finger.

Employee was helping move a heavy motor base when his left pinky finger became pinched under the base.

Struck against stationary object

Emp was laser aligning coupling on Lang Main Line #17 drive. While tightening the bolt for the dodge coupling guard, the wrench slipped and left elbow hit the corner of motor base adjustment bolt.
