Inhalation of radiations, caustics, toxic and noxious substances

After using a gas powered chop saw to cut cement, EE stated was not feeling well and wanted to go home. Once home, EE felt the dizziness, headache and nausea could have been caused by inhaling exhaust from the chop saw and sought medical treatment.

Employee was getting out of a vehicle when upon twisting felt pain in the lumbar region of back. Please note: Employee had surgery on back within the last 12 months.

Employee was roof bolting and stepped in a slightly uneven cut and hyper extended left knee. Emp. went to Dr. no restrictions or meds. On 5/17/2017 emp. had surgery for torn Meniscus. Emp on restrictions and pain meds as of 5/18/2017.

Caught in, under or between running or meshing objects

EE was using a hand-held air chipping hammer to remove build-up on the Granulator. As EE moved left hand down the towards the base of the chipping hammer, left middle finger was pulled into the tool & pinched. The employee was wearing leather gloves. EE taken to ER & treated for fracture to left middle finger with sutures. Pain Med was given & emp placed on restrictions.

Over-exertion in lifting objects

Employees just finished cutting a piece of 10" channel with a torch. EE was repositioning the channel for next cut when EE felt pain in lower back. Pain identified as a muscle strain.

Over-exertion in lifting objects

Emp was holding cage door on the bottom deck of #5 shaft mancage. Another emp was prying on the bottom of it to lift the door out of its hinges. The emp strained back when the door was dislodged from the hinges. Emp stated they did not want to see a dr at time of injury. On 3/28/2016 emp requested to see dr and was placed on restrictions for a strain to right shoulder.

Fall to the walkway or working surface

Employee tripped on an elevated piece of the roadway. The asphalt had recently been removed due to trenching work that was done. Back fill had eroded away due to high winds causing an approximate 2-inch elevation change in the walking surface. Taken to ER, has fracture to right shoulder. Given prescription meds and placed on restrictions. Surgery pending.

Fall onto or against objects

Emp was preparing to wash out the bed of the battery car. Emp was standing on top of the battery car and fell 62" to the ground. The employee struck right eye area on an orange traffic cone lying on its side. Emp taken to ER, has fx to eye and had stitches. No restrictions.
