# 5 hoist was lowering workers to 850 level, when left brake on hoist drum slowly engaged. Workers were stuck 9 feet above 700 landing. Low pressure on left brake hydraulic system caused brake counterweight to drop and engage brake. Workers communicated to hoistman and climbed down to lower cage and exited to 700 landing at 6:56 AM.

#5 Hoist down and not running related to a track limit switch. Hoist #2 and Hoist #4, both man hoists, still operational. This is only notification of our #5 hoist being down. Hoist back up and running as usual at approximately 2:00 pm on 9/18/12.
Two employees were working in No. 1 Shaft. They were down approximately 400 feet washing in shaft. They heard two loud noises above them and they moved close under the bonnet when the bonnet was hit by falling objects. The first knocked the bonnet loose on one side and the second hit the injured employee on the shoulder. It also broke the back of his hard hat. Emp taken to ER.
Emp.'s were replacing cutter motor on CU119 the new motor was on a trailer they were in the process of removing the clutch cover on the new motor when the last bolt was taken out the cover fell pinching the employee's left middle finger between the cover and the side rail of the trailer. Emp. has a fx to his finger.
4 employees were staging 54" return rollers for replacement on the #7 Lang Main Line belt. One employee was pushing the roller to the side of the tractor bucket, then another employee would lift the roller and slid the roller out the side of the bucket. The roller inadvertently shifted in the bucket and the employee's finger was smashed between the bucket and the roller.
While trying to move a piece of pipe into position the pipe moved unexpectedly caught employee's hand between a beam and the pipe
Three employees were working on changing oil on #1 bird. Superintendent arrived as this was in progress witnessed the buckets melting. Superintendent asked employees to stop and to barricaded area off, decks below. When superintendent went around the bird to get water hose, employee was using water hose to wash oil from left side of face.
Emp. was driving up the slope underground, he fell asleep and drove under the conveyor belt. Emp. was taken to the ER and received 6 total stitches and placed on restrictions until 6/11/12.
Emp had installed a rubber strip.The employee was making a final adjustment to the rubber skirt to extended down onto screen because metal lip was worn. The employees left hand moved along the feed lip to adjust rubber strip on worn feed lip. Metal feed lip cut into the employees leather glove. The employees left index finger was cut as a result. Emp. received 4 stitches.
Emp.was hanging up miner cable another emp. was helping out with the miner cable. Emp. stepped away and took one step and felt pain in his lower back and also felt a sharp pain down left leg and a twitch in his right leg. Emp. was taken to company Dr. and given a prescription muscle relaxant and placed on restrictions.
Emp. slipped or tripped while pulling an underground cable. He fell into a trough on his right side hitting his head, hip, and knee and said he felt like he twisted his ankle. Emp. taken to Dr. placed on restrictions, able to work job with restrictions. Confirmed torn meniscus on 2/10/12 and reported to MSHA.
While using a sledge hammer to drive in a I-beam employee miss and mashed his thumb between the beam and hammer handle.
Emp. was working on level XMTR, when a bee got into outer shirt, stinging him on the left side of hip, at belt line. Then into right sleeve stinging him on the right arm. Emp. taken to company Dr. and given 2 antibiotics for infection.
Employee was in the process of replacing the east primary cyclone feed pump when the employee was stung by a bee in the upper right arm area. Employee went to supervisor to report bee sting. Emp. taken to company Dr. on 8/16/12 and given two antibiotics for infection.
Employee was hooking up air impact into airline. The employee used a wrench to tap on the air line to clear out brine, stated that he was trying to get debris to come out of the fitting before hooking up impact wrench. He tapped on rubber grommet to loosen material.. Valve was in the off position when it broke. Employee closed a blocking valve upstream to isolate air.
Emp. was working in area 8 when a ground fall in the old working annihilated the area. Crew had to travel through the salt dust to evacuate the mine. Emp. taken to company dr. on 3/28/12 and given a prescription medication. No restrictions given.

Crew was tramming MBC's up to miner. Man on back MBC was almost dollied out and was on bad floor causing MBC to lean. He heard a noise from behind him and used his left arm to raise up and look back when he felt a pop in his elbow. Emp. given prescription pain med on 2/21/2012.
