# 5 Hoist electrical fault causing hoist to not operate. PLC control enclosure had a temperature of over 120 Degrees. Cabinet was cooled down and hoist was operational at 9:00 PM. Two other hoists were available for U/G miners to enter and exit the mine when the hoist was down.

Emp. was riding down the shaft and felt a pinch on his right leg. He later had swelling, irritation, itching and burning. Emp. went to ER related to bug bite. Emp. instructed to take 2 days off work and given antibiotics and pain meds.
Six employees removing a falk coupling. The broken hub in the falk coupling was loose inside of the coupling hat. Employees had a plan to stay out of the line of fire. Emp. that was injured got in the line of fire. The broken hub inside fell out and rolled onto employees right leg. Emp. received a laceration and 2 puncture wounds. Emp. taken to the ER.
Emp. parked ram car and exited vehicle. He stepped off the car onto some loose muck, his right foot twisted and he rolled his right ankle. He fell to the ground. He was taken to ER and placed on restrictions and given a prescription medication.
Emp. working with 6 other employees taking down cable from J-hooks and dropping the cable on the ground. The weight of the cable bent the employees wrist back as it fell to the ground. She did have a fractured wrist and had surgery to repair. She is currently on restrictions.
Emp. welding when he felt a piece of hot slag fly into his welding glove on his left hand. Emp. removed glove and said he saw a small red spot on his finger where he had been burned. Emp. continued to work. Emp. reported injury on 10/5/11 because left hand ring finger was red and swollen with drainage. Emp. taken to company Dr. and given oral antibiotic and restrictions.
While using a pipe threading machine employee got his hand caught between the pipe and the machine, causing lacerations to right hand between thumb and forefinger.
Emp. was assisting 2 other emp. lifting a hand rail when he felt a crunch in his lower back. He had pain/discomfort and was taken to the company Dr. Emp. was given 2 prescription medications and placed on restrictions.
Emp. was changing out a gearbox on a drive belt. Emp. smashed finger between gearbox and metal table. Right hand thumb was smashed. Emp. taken to ER and received 3 stitches to open area. Emp. is currently on restrictions.
Emp. was using a 24" pipe wrench to turn motor sheave to move output shaft of reducer. Emp. got his rihgt hand index finger caught between the wrench and bottom half of the guard. Emp. finger was smashed and cut. He will have surgery to repair on 08/24/2011.
3 emp. were replacing a broken cable. All emp. were on their knees on the deck in front of spring box assembly. One emp. was using a 3' bar to hold the cable bolt in place. Another emp. place the wrench on the nut on top of the cable bolt to loosen it. As the emp. pulled the wrench the bar came out of the spring box and came into contact with the other emp. left knee.
Employee was prying a bent return roller hanger. Since the belt was up against the ribs he had to stand next to the structure to get the hanger off. The scaling bar slipped, causing the employee to fall forward into the belt structure. Employee's right ear hit the structure causing a laceration.
Emp. was walking with a fluorescent bulb. She set the bulb down and walked down one step she attempted to pick up the bulb and finish the next two stairs; however fell and stated she could not remember what happened. Emp. taken to ER and did have a fracture to her left leg.
Emp. cleaning trash out of compactor. Emp. started noticing pinching and itching to right upper inner leg. When he looked there were 2 red hard areas. Appeared to be bug bites. Emp. taken to Industrial Health Services. Antibiotic given and placed on restrictions.
Emp. was raising the lid on the battery to let it cool, the latch to hold the lid was not secured and fell and crushed middle finger on his right hand. Emp. taken to ER.
Emp. was taking off metal cover off the granulator feed drag with three other employees. Cover fell and crushed emp. thumb in between the cover and a metal beam. Emp. taken to ER.
Employee walking down steps. When she stepped on the last step, she stepped on a water hose, which rolled out from under her, causing her to fall.
Emp. was coming off ladder, stepped off last rung and twisted his right knee. He was taken to the dr. and placed on restrictions. He did state he had injured this knee in the past.
Emp. riding in a mantrip as a passenger. Driver of mantrip hit the side of the rib. Vehicle turned slightly sideways and emp. hard hat and safety glasses flew off his head. Emp. denied need for medical attention; however the following day he had left shoulder, neck and back pain. Emp. taken to the dr. and placed on restrictions.
Emp. and another emp. were cutting portions of the skip hung up in #1 shaft around the 900 foot level, when the skip suddenly came free and jumped upward approx. 4 feet, pinching emp. left foot between the shaft wall and the skip.
Emp. was swinging a 2 lb pin hammer to remove a link froman MBC conveyor chain when he felt a pain in his right elbow. Emp. requested medical attention. He was taken to ER and given restrictions and pain medication.
Emp. was working on West Tails Screen applying silicone on a crack between the poly decks and discharge lip on screen. When he finished he turned around to climb out and twisted his right knee in the process.
Emp. was positioning valve cover lid. Emp. and contractor were rotating the plate back into position. Contractor thought the emp. head movement was a signal to let go. The lid fell into position crushing the emp. right middle finger between the concrete and valve cover. The emp. received 19 stitches to close the wound.
Emp. eye was wet from perspiration. When he was wiping the sweat from his eye, he inadvertently wiped sylvite ladened grease from his cotton glove into his eye. Emp. stated that his eye felt irritated and was watering, he denied need for medical attention. On 1/20/11 emp. taken to the doctor related to eye irritation/redness and drainage. Emp. placed on restrictions.
Emp. was driving CU106 Boom Rig around a rib at main shop. He hit a pothole which caused the air ride seat to hit bottom and jar his lower back. He said he felt no immediate pain. Later in the day the pain increased. When he went home for the weekend he started taking Oxycontin. Injury reported on 1/10/11.
Emp. operating front end loader when he felt a sting on his left shoulder. Emp. taken to ER because he was feeling sick. Emp. given an antibiotic and Tetanus injection. No restrictions.
Emp. was spooling off new hoist rope on cable reel. Hoist rope socket got caught on axle of reel machine. Emp. lifted spool to get cable unhung. Emp. reached to grab rope with hand and rope sprung out. Wire rope hit emp. in upper lip. Emp. upper lip was cut and left front tooth was chipped.
Emp. was changing a battery on a battery car. Emp. was nipping in car cable to #2 battery to put in use and emp. was shocked. Emp. went to ER and was given prescription medication, no restrictions.
Emp. noticed red, swollen, red irritation on the left side of his navel on his stomach. He stated he was welding on ventilation fans the day before 8/4/11 and during the process a chip of slag or a "pop" of hot metal entered his shirt and he felt a burning sensation close to his navel. He went to the dr. and received an antibiotic. Emp. did not report injury until 8/5/11.
Emp. cut back of his hand between the knuckle on the ring and middle fingers (left hand) while blowing the grinder table area off. Emp. cut his hand on a duct opening behind the grinder, the flashing around each duct opening has sharp edges. Emp went to the ER and received 2 stitches. No restrictions.
Emp. stripping away PVC coating from electrical conduit with a pocket knife in his right hand when it slipped and he cut his left thumb on his left hand.
