Electrical service power from provider was off due to electrical storm. 2 other hoists were available for miner evacuation. Power was restored at 4:00 am on 8/20/05.

#4 Hoist down from 0600-1600 due to work being done on Safety Circuit-Lilly. Hoist returned to operational status at 1600.
Phone and bell signals out on #5 shaft due to lightning strike. Hoist itself operational, but no communication.
Emp was adjusting wiper bracket, using a sledge hammer and standing in an awkward position. He felt pain in his right shoulder while hitting wiper bracket with hammer.
EE WAS CLIMBING DOWN LADDER. THE BOTTOM OF THE LADDER SLIPPED. EE STUCK HIS FOOT BETWEEN RUNGS AND TWISTED RIGHT KNEE. TOP OF LADDER WAS TIED OFF.
Emp was walking out the South entrance of the crusher, where there is a dirt ramp exiting the building. The dirt ramp has a 6" drisco pipe running through it for drainage. The water had undercut the ramp alongside the drisco drain. As emp walked down the ramp, the area above the pipe gave way, causing her right leg to fall into a hole approximately 12-18 inches deep.
Emp was walking to the front of his tractor when he tripped on an expanded metal tail roller guard that was lying on the floor. Emp dislocated shoulder attemtping to stop his fall.
Emp bent over to pick up wiper bracket and felt pain in his back. Initially refused treatment, sought treatment 6/30/2005 with company physician resulting in 25# lift limit restriction.
Employee was cutting left pass in BT 53 with about 10 ft left. The right and center pass were in 30 ft. The operator was standing about 10 ft from the cutting head (to the right and back). He stated that a rock came from the continuous loading arm, striking him on the right lower leg. The crew treated the cut and he received five stitches at the Emergency Room.
