Accident type, without injuries

A power outage occurred at 5 Shaft at approx 3pm on 9/2/13. Recloser #1 at MOS #3 sub tripped and locked out; this is suspected to have occurred due to storm activity in the area. #5 Hoist lost power just as it began hoisting 14 employees. Power was restored about 2.5 hours later. After hoist checks were completed all employees were then safely raised to surface.

Over-exertion (Not Elsewhere Classified)

During rounds at the crusher at the 1026 belt, the employee's ankle rolled while walking on the cat walk. There was nothing in the walkway and no other hazards present. Taken to company Dr on 8/26/13 and placed on restrictions.

Fall from machine

Emp. was loading an excavator and he went to get off and the equip. started to tip over and emp. jumped off and landed on his left heel. Emp. taken to the company Dr. He has a hairline fracture and was placed on restrictions. Emp. will return to work on his normal shift 7/24/13.

Struck against stationary object

Emp was carrying a tool bag. The tool bag was in front of him about chest high which limited his visibility. He made a right hand turn. When he turned he stepped on a timber 6"x6"x3' and twisted his right ankle. He requested to go to the dr on 7/25/13 and has 2 fractures. He was on days off after the fall and requested not to go to the dr. at time of injury.

Struck by falling object

Emp. was getting impact out of toolbox. The lid can be laid flat when opened to eliminate chance of falling. Employee left lid in upright position and when he reached for impact, the lid came down on employees left hand small finger. Employee did have gloves on at time of incident. Emp. taken to ER approx. 10 stitches to finger, fracture to tip and restrictions given.

Struck by... (Not Elsewhere Classified)

Emp. was hammering a conduit clip flat. As ee approached emp. ee felt something hit his thumb on his right hand. EE looked at his hand and there was an 1/8 cut above the knuckle on his right thumb. He went to the dinner hole and determined that a piece of metal was stuck deep in the cut and he could not dig it out. Taken to ER and metal removed by surgeon.

Emp. was on Battery Car. He became unresponsive at approx. 1:50 pm. his hard hat fell off and another employee ran to help him (Facundo Baca) Emp. was taken out of the mine on stretcher to be taken to ER, and was pronounced dead at 4:30 pm by OMI on the Hobbs Hwy in route to town.
