The incident occurred in a parking area. The walking surface is paved. At the time of this incident the walking surface was dry, flat, and well lit. There were no safety hazards in the area. The employee's knee gave out with no assistance from any onsite hazards. The employee fell on left knee and was later discovered that left tibia had fractured and required surgery.

Employee was traveling underground in a mantrip checking ventilation line on the main travel way. The vehicles passenger side front fender came in contact with pillar of mine causing it to come to a sudden stop. As a result employee taken to hospital.
Employee was exiting a vehicle and felt pain in left knee. Employee went to the ER on 1/7/22 related to increased swelling to the left knee. Employee was given a prescription med and placed on restrictions.
Employee was removing motor pulley, the puller came off & hit EE in the abdomen. Emp. was evaluated & released back to work without restrictions. MRI was ordered. Emp. was placed on restrictions related to hernia to abdomen on 9/15/2021.
Emp. was on the bottom floor of the KMAG Screening Plant planning clean up activities when EE was struck on the left shoulder/neck by a piece of product buildup falling from above. Emp. taken to ER and placed on restrictions and given prescription meds.
Emp was descending the stairway from the electric shop breakroom and felt a pop in EE's knee. Emp. was taken to the company clinic on 7/27/21 and was placed on restrictions and given orders for physical therapy.
Emp. was carrying two buckets of miner bits each weighing approximately 26 pounds to the mantrip. The employee tripped and lost their footing causing employee to drop bit buckets and in the process twisted and strained back. Emp. sought medical assistance on their own on 7/27/21 and was given 2 injections and a prescription for a muscle relaxer.
Emp. stepping down ladder from stacker underground and right knee popped, a few min. later emp. felt pain. Emp. had MRI on 7/26/21, working without restrictions. Emp. was ordered physical therapy visits on 8/5/21 and continues to work without restrictions.
Employee tested positive for COVID-19. After investigation it was determined on 8/04/2021 that this was a workplace exposure.

Emp was operating a ramcar. Emp stated the articulation jack was bypassing & had to keep pushing & pulling the lever to prevent the bed from contacting the ground. Emp started to feel soreness in the right elbow on 1/10/2021. The emp was tasked with using an impact wrench & the pain in the elbow increased. No restrictions given however physical therapy ordered on 3/10/2021.
