Struck against a moving object

Employee was descending down a coal haul road when he lost control of his coal truck and went over an embankment.

Struck by falling object

Employee was helping lift a saddle off a 992G when the saddle came off the pin falling on his right foot. Employee's foot was between the saddle and the outrigger of the boom truck.

Caught in, under or between a moving and a stationary object

Employee was changing a bogie on a D11 roller frame. Employee started to get up when the other mechanic moved the bogie, catching the employee's fingers between the bogie and the frame.

Fall from machine

Employee claimed that he hurt his knee on Saturday 07/23/05 but didn't tell his foreman until Monday 07/25/05.

Employee was on the top of the powder truck when he slipped and fell causing employee to sprain his ankle.

Struck against a moving object

Employee was pushing rock with a dozer when the dozer lost traction and slipped back. When the dozer caught traction, it jerked the employee's back.

Employee was working on hydroseeder truck when he fell to the ground injuring his left shoulder and collar bone.

EE was pulling impact wrench from beneath a D11 dozer when the coupling came loose allowing the air hose and coupling to strike the EE in the nose and right eyebrow area.

Struck by flying object

Employee was helping mechanic beat a pin out of a 777D truck strut when a piece of metal broke off the hammer striking the employee in the thigh.

Employee was helping beat a bushing off a gear when a piece of metal broke off, striking the employee in the left forearm on the ulna side.

Struck against a moving object

Employee was tramming dozer towards parking lot when he was blinded by the sun. The dozer dropped over a hump, hitting the blade and causing the dozer to stop abruptly. EE hit his head on cab of dozer (11 stitches to forehead.)

Fall onto or against objects

Employee was walking down slope pulling hose for the hydroseeder when he slipped. Employee caught himself with his right hand on a rock and cut his thumb area. He received four stitches.
