Employee was welding on pipe when a hot piece of metal melted employee's glove & burnt right index finger, later on that morning employee was smacking down a screen with a hammer when employee hit the same finger with the hammer, employee did not report to Foreman until 11-16-19, had surgery on finger & is still in Hospital.

Employee has strained back from continuously exiting and entering service truck.

Struck by flying object

Employee was cutting a piece of rebar, took the torch to blow away dust and metal shavings, when it blew back in employee's face getting into right eye.

Over-exertion in pulling or pushing objects

Employee was chaining down backhoe on lowboy trailer, when employee slightly bent over to pull chain binder, employee felt a sharp pain in lower back.

Employee was putting metal fence post in hole when employee slipped and left leg went into hole up to employee's hip, bruising leg and hip.

Over-exertion in wielding or throwing objects

Employee was shoveling mud that was built up against the belt, employee was throwing off of top of tower and twisted lower back.

Struck by flying object

Employee was working on segments on D11 Dozer when EE was hitting with hammer a piece of metal flew off and hit EE on right shoulder embedding itself. Worked until EE had surgery on shoulder 7-30-19.

Employee was stepping up on D10 Dozer to check oil for starting of shift when EE twisted left knee.

Fall from machine

Employee was climbing up ladder on 930 Loader to start it, when the ladder broke and ee fell skinning shin below left knee, ee just went to a doctor for this injury 3-25-19.

Employee had gotten off grease truck, was walking and step in a small dip in ground and turned left ankle. Just found out ee has been off from work for this injury.

Fall from machine

Employee was exiting D11 Dozer when they stepped on push arm step which had frozen mud on it, their foot slipped and they fell to ground hurting right shoulder and knee.

Fall from machine

Employee was changing a hydraulic filter on a D-11 Dozer when employee's wrench slipped, employee fell off dozer on ground cutting right side of face above ear, receiving (5) staples.
