Employee was unloading a box of floor sweep when something from the top of the box blew into his eyes.

Caught in, under or between a moving and a stationary object

Employee was trying to get a headache ball loose that had caught on the boom, when the ball fell, catching the employees arm between the ball and a piece of steel.

Fall from machine

Employee was climbing up the ladder on a 777D haul truck, when he stated that his back gave out, causing him to fall about 5 feet to the ground.

Employee was walking in the pit when he stepped in a drill hole, and twisted his knee.

Struck against a moving object

Employees was pushing spoil with a dozer. The employee went forward over the pile of dirt and when the dozer fell over the dirt the blade hit the ground, jarring the employee.

Employee was reaching a wrench under a truck to another employee, when the employee straightened up, he felt pain in between his shoulders.

Employee was pushing in a shot, when some debris fell from the highwall striking the dozer window. The glass from the window cut his arm and some glass went into his eyes.

Employee was dumping a load of coal when the truck turned over, causing the operator to hit the drivers door with his hip and back.

Employee was cutting brush on the berm when he slipped. As employee fell forward, he put his right hand on the chain, cutting his hand.

Employee caught his jacket and shirt on fire, causing burns to left arm.

Employee was checking grade on a ditch when he fell on a rock striking his side. Employee was taken to the hospital and recieved 2 or 3 stitches.
