Over-exertion in pulling or pushing objects

Employee was sowing grass with a hydroseeder hose when he pulled the hose and strained his lower back.

Over-exertion in lifting objects

Employee was helping a mechanic pick up a metal plate from a D11 dozer when a sharp pain hit in the middle of his back causing him to fall to his knees.

Employee was exiting his work truck when he stepped on a rock and caused him to fall forward thus twisting his back.

Employee was starting a water pump and went to jump from pump platform back to bank of pond, he slipped and hit his head on the bucket of an excavator that was parked there.

Employee was dumping a load of rock when the truck started to turn over, he tried to let the truck bed down but the truck turned over. The truck flipped four times before coming to a rest on all 6 wheels.

Employee was riding with dayshift foreman when they stopped to look over into a hollowfill. Employee stepped onto a berm to look at the hollowfill. As the employee was stepping down the berm, he stepped onto a rock, causing him to lose his footing.

Employee was backing a 777D truck to the dump when he backed through the berm causing the truck to overturn.

Employee was running a loader when he stated that he hurt his back. Employee refused any first aid help or transport in an ambulance to the hospital. The employee left the job against the advice of the foreman.

Employee was moving a light plant when he walked to his truck, stepping on some ice causing his feet to slip out from under him causing employee to land on his back and back of his head.
