Fall to the walkway or working surface

Employee was walking through warehouse toward her office, slipped, fell face forward on concrete floor hurting her right shoulder.

Employee was traveling around a curve when he stated that it felt like the truck set down and it turned over onto his right side

Employee was sowing grass using the Honda 350 4-wheeler when he was going around a mud hole when the 4-wheeler hit a tree and turned over hurting his left side and shoulder. The injury date was 4-23-15, did not go to Doctor until 5-12-15. He was put off from work to do tests.

Struck by flying object

Removing exhaust clamp - it broke and was slung by air ratchet cutting left finger

Employee was helping replace a 14"" steel pipe and was moving tools out of way when his foot slipped and twisted his right knee.
