Struck against a moving object

Employee was reclaiming highwall with dozer when the dozer flipped and rolled several times landing in the road. Employee was unconscious with labored breathing when they got to him.

Employee was removing rock from rock crusher when he pulled muscle in back.

Employee was hydro seeding on a hill and was pulling hose when he strained the lower part of his stomach.

Employee was putting radiator core in D10 Dozer when his feet slipped throwing him between the stabilizer bar and grill of dozer hurting his shoulder/elbow. He didn't miss work until 12/13/2011.

Employee pulled hose up steep hill, and was coming back down when his feet slipped and he fell 15 ft down and then tumbled another 15 ft down.

Employee was splicing wire for CB radio on a 924 Loader when his knife slipped, and sliced his right thumb. Employee went to doctor on 9/27/2011.

Employee was throwing metal out of the stoker plant building when he twisted his lower back.

Employee was changing fuel filters in 980G loader when the hood fell on employees arm hurting right wrist.

Employee was driving coal truck, when he went into the ditch causing the truck to turn over and the coffee thermos came up and hit the employee in the upper lip.

Employee was being loaded by a loader when a rock fell from the loader bucket and hit the bed of the truck jarring the employee's back.

Employee was working on drill when he started to get up, his foot got caught and he twisted his left knee.

Employee was getting off of drill using the ladder when he stepped off onto an unstable rock causing him to twist his knee

Employee was pulling water hose out of water when he pulled something in his abdomen. Foreman was told but employee said he was okay. His abdomen continued to bother him so he went to the doctor on 9-12-11.

Employee was putting batteries into a 993k loader when he twisted his knee the wrong way causing his knee to swell and cause pain when he bends it. He didn't go to the doctor until 08/15/2011

Employee was hauling mud on Volvo truck when he went across ruts in road and jerked the cab of the truck hurting his lower back. Employee didn't tell foreman until next day 7/29/11 and did not go to ER until 7/30/11 and is not going to his regular doctor until 8/1/11, he then came back to work on 8/3/11 and left early to go back to the ER with back pain.

Employee was on the hydroseeder crew, was pulling hose up a steep hill when his feet slipped out from under him causing him to fall on the right shoulder.

Employee was tramming a track loader on roadway when he ran over an uneven portion of roadway causing it to jar his lower back and legs.

Employee was loading a rock truck with a 993 loader when the bucket of the loader dropped hard on a rock and jarred his middle back.

Employee was returning to the pit when for some unknown reason, the employee drove over the berm. The employee traveled approximately 144 feet down the embankment before the truck came to rest on its right side.

Employee was getting off a 777 when he slipped and fell injuring his back and left shoulder, nothing was broken.

Absorption of radiations, caustics, toxic and noxious substances

Employee's trucks air conditioner was being repaired the morning of 4/25/2011, employee states Freon sprayed into the cab and caused irritation to his eyes.

Employee was getting off the truck at quitting time when he slipped and fell hurting his back. Employee did not tell his shift foreman that night, instead he called back and told the day shift foreman the next day.

Employee was cleaning up area after a storm with a skid loader. He kept twisting his neck to back up then go forward. Developed pain in neck. Employee didn't immediately go to the doctor.

Employee was using a pry bar to remove a transmission out of a 777 truck when his hand slipped off the pry bar and his struck the 777 truck.

Employee was working on belt when he slipped and fell and hurt his upper back. This happened on 3/30/2011, but he didn't tell his foreman until 7/25/2011.

Employee was driving his parts truck on hwy 160 when the power steering belt broke and the truck veered into the guard rail. This caused EE to hit his left wrist on steering wheel.

Employee was bent over moving coal and dirt away from core holes with a rake, when he developed continuous pain in his neck. The pain worsened on 4/04/2011 and he went to the doctor.

Employee was cutting the tip off a silicon tube when the knife slipped and stabbed himself in the right leg above the knee.

Employee was stepping off D11. Stepped from push arm to the ground and fell on the ground flat on his back. Employee didn't not miss work until 3/28/2011,which was when he was put on night shift he went to doctor on 3/29/11 and the doctor told him to take off two weeks.

Employee was unstopping a rock crusher when he went to move a rock, and his foot fell through the grizzley, he went to catch himself and hurt his left hand.

Employee was walking down catwalk of excavator when the catwalk slipped down about 5 inches, jarring the employees knee. Employee did not go to the doctor until 4/7/2011 when his foreman made him due to severe pain.

Employee was driving the 777 down a hill, when he stated the brakes failed and the truck overturned. The brakes were checked after the accident and seemed fine.
