Struck against a moving object

During over the normal use of the dozer over a period of years, employee claims that his back has become so painful that he is going to the doctor.

Employee was driving a 777 rock truck when he drove through the berm and turned over. Employee complained with shoulder pain.

Employee was working on the lights of a dozer when he slipped and fell, twisting his knee.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was dumping a 777 haul truck, when the fire extinguisher became dislodged and fell onto the frame of the truck. When the employee lowered the bed of the truck, the fire extinguisher was crushed, filling the cab of the truck with the contents of the fire extinguisher.

Employee stated that his back and hip were hurting causing his left leg and hip to hurt. Employee stated that shifting his weight to his right leg caused his knee to swell. Employee was admitted to Harlan ARH hospital to drain the fluid off the knee.

Employee was stepping off a truck when he slipped and twisted his knee. NOTE: Didn't think employee was going to miss any work at the time.

Employee was changing filters on a 992 loader when he felt a burning sensation in his naval area.

Struck by falling object

Employee was removing the belly pan when it fell striking his leg. Took employee to doctor and released him to go back to work. Infection got into the leg and employee had to have the leg lanced and the infection cleaned out. He had the surgery on 04/26/07.

At 08:40 am this morning, Employee was found slumped over in a dozer by another employee. He was pronounced dead by the emergency room Doctor from a massive heart attack at the Harlan A R Hospital.
