Employee was dismounting dozer when EE stepped off track onto push arm, slipped and sprained left ankle.

Over-exertion (Not Elsewhere Classified)

Employee was helping foreman set a pump near a pond when EE twisted their right ankle, went to Mary Breckingridge Hospital ER and they said it was a sprained ankle.

Fall to the walkway or working surface

Employee was on catwalk at Washer (scalping screen) when EE was working the slack out of the come-along when EE slipped on mud landing on right arm.

Over-exertion (Not Elsewhere Classified)

Employee originally got hurt 5/23/14. EE reported to safety that ee had back problems 1/5/16 and that it stemmed for the earlier injury

Struck by flying object

Employee was taking cylinder off a D9T when EE was taking bolt out of light bracket a pin flew out of wobble socket, struck left hand cutting it. EE went to ER in Pineville, Ky. received (5) stitches and back to work 03-18-16.
