Fall from machine

Employee was climbing off grader when the wind blew the grader door hit employee in back knocking employee off grader onto ground, landed flat on back, knocking the breath out of employee.

Employee was exiting dozer, when employee's right foot slipped off step landing on employee's ankle.

Employee was exiting Dozer when EE's left foot slipped on step and twisted left ankle.

Fall onto or against objects

Employee was walking on High Wall Miner, lost footing, stumbled and tried to catch self, hit the stair on the Miner injuring right side back and ribs.

Employee was putting in transmission in 40-ton Truck, when the Come-Along slipped pinning the employee's left arm against frame of Truck.

Employee was operating 777 Rock Truck when they hit a hole in road jarring lower back and felt numbness in left leg.

Contact with hot objects or substances

Tech was using a torch to cut out a tank on a 993K loader. Sparks and spurs came off as tech was cutting landing on pant right pant leg. Tech didn't realize pant leg was on fire until it had burnt tech. Received minor burns on finger tips as tech was putting out the fire on pant leg. Tech had taken off gloves to pat out fire.

Over-exertion in lifting objects

Employee was loading mulch on Hydro Seeder Truck when they felt a sharp pain in lower back,

Absorption of radiations, caustics, toxic and noxious substances

Employee was refueling 777 Truck. When EE took nozzle off of fuel tank, the fuel sprayed out & hit employee in face, the O ring hung on quick connect on fuel tank.

Contact with hot objects or substances

Employee was burning brush pile with an excavator, when wind caught the fire and blew it in the cab with the employee burning EE on left side of face, elbow, leg, & hand.
