Employee was drilling a hole for an anchor pin on the Highwall miner when the anchor pin fell striking the employee on left foot

Employee was taking D11R Dozer up on shot when the face of the shot broke causing the dozer to turn over hurting right thumb and spraining left ankle.

Employee was heading down the hill in 785C Rock Truck after unloading, road was wet after raining, lost control, started going side ways and hit a rock wall and turned over.
