Over-exertion in lifting objects

Employee was helping set up a 55-gallon barrel of water when he lifted his side of barrel and felt a burning sensation in left side of stomach (groin). Employee got hurt 12-28-12, has worked all the time, is having hernia surgery Friday January 25th, 2013 will be off work 6 weeks.

Struck by flying object

Employee was using a chipping hammer to change metal screens on a vibrator when a piece of metal flew into right eye around safety glasses.

Fall onto or against objects

Employee was helping mechanic work on dozer when a wrench fell between push arm and front track of dozer, when trying to get wrench, he slipped and cut left thigh on track.

Struck by flying object

Employee was helping change air filter in loader when the wind blew dust in his left eye.

Over-exertion (Not Elsewhere Classified)

Employee was hydro seeding hill, when coming off hill he stepped in hole and jarred his left knee.

Struck against stationary object

Employee stepped between rocks in spillway when he twisted his right knee area. Employee is just now having surgery on knee (9/30/2014) expected to be off work 4 to 6 weeks.

Employee told Foreman that he hurt his back, When asked by the Foreman, did he hurt it at work, the employee stated that he did not hurt it at work and he had turned it in on his health insurance. We later received a letter from a lawyer stating that the employee hurt his back at work.

Over-exertion (Not Elsewhere Classified)

Employee was dismounting dozer for lunch, when he was getting off he twisted his lower back. He did not go to Doctor that day, but today (10-22-2012) he went to Dr's office in Harlan, Ky.

Fall onto or against objects

Employee was looking for an oil leak on Core Drill RD-16 when he leaned across hydraulic tank, slipped and fell hitting on his left side. He went to the Pineville Community Hospital. He has (1) One broken rib.

Bodily reaction, (Not Elsewhere Classified)

Employee was on floor board of D-11 Dozer looking for an oil leak, when he raised up, he felt a sharp stab in his lower back. He was taken to the E.R. in Pineville and will be seeing his regular Dr. today.{9/25/2012}

Employee was in a meeting sat in a chair, the chair leaned back and jerked his back. He went to see Dr. on 9/24/2012.

Employee was connecting (2) air hose together when one slipped out of his hand and struck him above his right eye.

Struck against a moving object

Employee was cleaning coal with a 930 loader when his bucket caught on a sand rock and he twisted his back. He kept working with back pain thinking it may go away, but the pain kept getting worse. On 8/13/12 he thought he was having a kidney stone attack and went to the ER At Harlan ARH. He then called and said it was his back.

Struck against a moving object

Employee was cutting a bench with D9 Dozer, went over edge of bench causing dozer to turn over on it's side hurting his left hip and knee. Employee was taken to Mary Beckingridge E.R. in Hyden, Ky. where he was checked out and released.

Fall to the walkway or working surface

Employee was trying to shove the forks together on a forklift when his feet slipped out from under him, landing him on his left side on the concrete floor, hurting multiple parts on left side. Employee went to the Doctor on 2/2/2012.

Employee was taking drain plug out of 980g loader using a wrench when he felt a pain in his right shoulder and bicep.

Employee was getting out of pickup truck when he shut his hand up in the door injuring his right thumb.
