Employee was walking up a sand incline leaving a pumping station when his right foot slipped and he fell forward as his right leg shot outward. He experienced tenderness in his right knee and thigh. The company physician referred the employee to an orthopedic physician. Outpatient surgery was performed to repair the injured knee.

Employee lifted a sixty lb. packing sleeve from the back of a truck. As he turned and started toward the work site he felt a sharp pain in his left groin area. He was treated for a minor muscle strain and returned to work. Later, the employee was diagnosed with a small hernia and surgical repair was performed on 10/5/05.
Injured employee was preparing to fuel an excavator. He placed the fuel nozzle on the catwalk by the excavator and then climbed up onto the catwalk. Once on the catwalk he lost his balance and jumped/fell 4-5 feet to the ground. An x-ray taken on 8/18 revealed a cracked bone. Employee was then placed on light duty status.
EE and a coworker were loading a pit car cable plug onto the back of a reel truck. The truck was parked on a dirt bank & the bed of the truck was 2-3' higher than normal. When EE began to lift the cable plug (approx. 50 lbs.) up on the bed of the truck, he felt a pop and then pain in his right shoulder & burning in his left shoulder.
Employee reported to his Superintendent that he had struck the side of his right hand against the window frame of the dozer as he was turning in his seat. He did not seek immediate medical attention. He was later evaluated by the Company Nurse and further by the Company physician, who ordered an X-ray which revealed a fracture at the base of the right fifth metacarpal.
Employee was changing out 8"" pipe on south side of Washer, requiring climbing and pulling on cables in pipe rack. He later felt discomfort in lower back. As a result from an Orthopedic consult, physical therapy was ordered for treatment.
Injured employee was climbing ladder on the shoe of the No. 3 dragline when his right foot slipped on the fourth rung of the ladder. Employee lost his grip on the ladder with his right hand and fell backward, approximately 3-4 feet, landing flat on his back.
Employee experienced hearing loss above 25dB in the left ear. The hearing loss was detected during the annual audiometric testing and confirmed by a second audiometric test.
