Employee was pushing rock into a draw hole on the rock pile using tractor. As he approached the edge of the hole, he misjudged the distance to the edge, which resulted in the hole collapsing causing the tractor to fall into the hole. The impact at the bottom of the hole caused employee to strike cab support with left shoulder.

Employee was removing the bottom bolts in 20"" matrix fitting (all other bolts had been removed). A 980 loader was holding the pipe. Employee was using impact wrench w/his R hand, L hand was supporting imp. wrench. As he removed a bottom bolt, it sheared, & another bolt pulled through the bolt hole. Pipe fell on employee's hand pinning his L thumb between the impact wrench & pipe.
Employee was replacing a damaged hyraulic line on a D8R tractor. Employee had to lie on his stomach to access the connection of the line. He then had to reach as far as possible to get his wrench on the bolt heads. As he went to loosen the bolts, his wrench slipped off the bolt causing a sudden jerk, straining his neck. Employee recently began physical therapy for treatment.
On 3/30/06 employee reported a gradual worsening pain in his right hand. On 6/2/06 employee was evaluated by an orthopedic physician and diagnosed with carpal tunnel syndrome in the right hand.
