Caught in, under or between (Not Elsewhere Classified)

Employee was applying light oil to the dollie rollers on #5 dragline. He was using an oil can with an approx. 18"" spout. He was standing on the shoe and leaning over to reach the rollers while the dragline was swinging. He then fell between the dragline shoe and tub, fracturing his pelvis.

Over-exertion (Not Elsewhere Classified)

Employee was repairing access ladder on #4 pit pump. The pump sled was covered w/uneven dirt/buildup, the dirt shifted and he felt a slight pain in his L knee. Reported to clinic on 5/1 w/discomfort in his L knee & received first aid treatment. On 8/20 he returned to clinic c/o continued discomfort. Referred to Dr. and on 9/12 was given restrictions and referred to ortho surg.

Struck by falling object

Employee was cutting a 600 - 800 pound piece of steel off of the center pin assembly of the #1 dragline tub. The steel was supported by a chain attached to a boom truck. When the steel was cut free, it shifted, striking the employee.

Struck against stationary object

Employee was walking in a stooped position inside the dragline tub and hit his hardhat on the walk gear. He was wearing the hardhat. He felt stiffness in his neck. The employee reported to the Clinic on 7/7/08 complaining of neck pain and was referred to a physician.

Employee was drilling holes in a metal substation housing, some overhead. He was wearing safety glasses. At 4:30 PM he felt irritation in his left eye & he washed his eye with water and went home. The next morning he reported the incident to his foreman & was seen by the company doctor. He was referred to an Optometrist and a small piece of metal was removed from his eye.

Struck by falling object

EE was removing a cross member under a pickup truck that was on a lift approx. 6.5' off the floor. The cross member has 4 bolts on frame and 2 in middle. EE removed the fourth frame bolt, the cross member fell to the floor, the center bolts were broken. The cross member struck EE on back of right leg as he jumped out of the way.

Flash burns (welding)

Employee & co-worker were welding flanges on pipes. Employee notice eye irritation after appr. 10 hrs. Irritation got progressively worse until he returned to work on 1/27 and received treatment from the company nurse. Prescription medication was administered.
