Over-exertion (Not Elsewhere Classified)

Employee was ascending stairs when left knee gave way causing employee to collapse onto the stairs.

Employee was dismounting D8 bulldozer when his foot slipped from bulldozer blade rail causing him to spin around hyper extending his right elbow under his body weight. He reported the incident and was able to return to work. He reported to work on 7/2 complaining of pain with restricted range of motion.

Contact with heat

Employee stated he was weak, cold and clammy. He had been working outside and was exposed to the heat and had suffered from stomach virus the previous night.

Absorption of radiations, caustics, toxic and noxious substances

Employee worked 6 pm - 6am on Monday, June 25. When he got home after working his shift, Tuesday, June 26 he noticed a rash on his hands. He is not sure what caused it. He reported the incident when he returned to work on his regular schedule Friday, June 29.
