Caught in, under or between a moving and a stationary object

Employee was cleaning out the log washer discharge chute with a cleaning rod while it was in operation. The rodding tool impacted a washer flight and started being pulled into the machine. This caused the employees left thumb to be pinched between the handrail on the secondary flume level and the rod.

Over-exertion (Not Elsewhere Classified)

Employee states repetitive use injury severe low back pain. No specific incident at this time. Referred to Orthopedic Institute. EE placed on restricted duty and placed out of work beginning 10/28/20.

Over-exertion in lifting objects

Employee worked numerous consecutive days and started having some pain in left hip. Could not identify a particular event that caused the pain. EE was placed on restricted duty and put out of work beginning 10/30/20.

Fall to the walkway or working surface

Employee was pulling water hose, nozzle came off causing EE to lose balance. EE fell landing on right side, felt pain in right shoulder after trying to catch themselves with right arm. EE was seen by EMT. They reported to clinic on 9/21 unable to raise arm, sent to orthopedic center and placed out of work.

Fall down stairs

EE was walking down stairs toward East & West bins when EE got to the last two steps EE fell rolling EE's left ankle.

Struck against a moving object

HEO operating D10 bulldozer on mill rock pile. Suspended air seat had leaked down. While backing tractor, felt abrupt jarring of back as EE traveled over change of elevation. EE reported to clinic on 4/20/20, treated on site, later had x-rays & MRI. Referral visit to Ortho 5/8/20 with restrictions placed out of work.

Fall onto or against objects

EE was going down stairs on w side of Float Plant after shift relief. EE made turn from 1st flight of stairs down to 2nd flight, EE slipped & both feet went over edge of top step. EE was able to hold on to handrail to prevent falling but bumped right shin in process. EE did not report incident immediately because it did not hurt at the time. EE will be off work effective 3/25

Over-exertion (Not Elsewhere Classified)

Employee was clearing area and got a tree on the track of bulldozer, when EE got out of cab to get tree off EE slipped off cleat on the track and experienced pain in right knee. EE's pain increased throughout the night. EE able to ambulate, but states that it is sore. EE reported to clinic 2/3/20, EE had surgery on 8/21/20.

Over-exertion in pulling or pushing objects

Employee was pulling themselves up onto platform & experienced a loud pop in left shoulder. EE cannot raise arm above shoulder height. EE was referred to The Orthopedic Institute and placed on modified duty beginning 2/12/20.

Employee was adjusting belts on de-oiler pump under float plant. Employee felt a slight burning in right eye, washed the eye at eye wash station but continued to feel a particle in the eye. Employee was taken to NFEC a piece of metal was removed from the eye. Employee was given a prescription medication.

Employee was en route to EE's job at the time of the vehicle accident. The cause of death was a medical cardiac event.
