Over-exertion (Not Elsewhere Classified)

Employee slipped in mud and twisted back while working on belts at A on west raw feed pump. Later EE dismounted dewatering barge, when EE stepped down ankle twisted and back pain increased.

Struck by falling object

Employee transporting 2 cylinder carts on forklift, removed first cart to push to staging area and 2nd remaining cart tipped over forwards and struck employee's right leg approx. 6-8"" above the ankle resulting in a break.

Fall onto or against objects

EE was walking under the washer and right foot caught in grating causing ee to fall. EE struck right back against concrete pedestal base as ee fell. EE was treated at onsite clinic & returned to work. On 2/18 EE had back pain and went to ER for treatment. EE was given pain meds and instructed to return to work on 2/21 (two days off).

Over-exertion in lifting objects

Employee was pulling and relocating dragline cable and experienced pain in right side of EE's back. Employee was placed on restricted duty beginning May 18, 2018.

Fall onto or against objects

Employee was going upstairs to measure raw bins, foot slipped on stairway which resulted in employee falling onto right leg just below the knee.

Fall to the walkway or working surface

Employee was walking beside the conveyor in the west tunnel when ee slipped on ice landing on ee right side.

Fall onto or against objects

Employee was working in field rolling cable up onto barge, right foot got caught in piece of rope which caused them to fall forward. EE's left knee struck a piece of angle iron that is welded on barge. The impact with angle iron caused laceration just below left knee cap. Employee received three sutures.

Caught in, under or between a moving and a stationary object

Area was wet, muddy and material in area was sticky.
