Over-exertion in lifting objects

Employee was walking carrying bucket & felt sharp pain in right shoulder. He set bucket down and had difficulty picking it back up due to pain. MRI on 06-06-14 revealed torn rotator cuff.

Over-exertion (Not Elsewhere Classified)

Employee operates a large dragline that utilizes a joystick for control and began noticing pain and numbness in his hand. Referred to a doctor 10/7/14 diagnosis is rupture tendon left hand.

Struck by... (Not Elsewhere Classified)

3 employees setting up an ACI to burn pile, needed help from excavator to extend the last 3'of burner pipe. Excavator operator while clearing path to ACI grabbed a small stack of limb debris in his way and pushed it into the pile. 4"" by 6' long limb sprung out of pile striking employee on the ground 25' away. Breaking forearm (Ulna) and c1 vertebra.

Struck by falling object

Employee was installing a bearing cap in a drag line. The bearing cap was being tapped into place when the 70# bolt dropped down and struck the employees hand resulting in an avulsion. This required 10 stitches to employees left hand.

Contact with heat

Employee was installing transmitters at the #5 tailings circuit booster when he became overheated and began to feel ill.

Over-exertion (Not Elsewhere Classified)

Employee had been working on a different dragline. He was using joystick in cab of dragline and having intermittent numbness in tips of fingers. Employee returned to company clinic on 4/21/16 with complaint of CTS symptoms of right hand pain (notification in July 2014). Employee being referred for evaluation.
