Struck by... (Not Elsewhere Classified)

Heavy equipment operator was driving excavator to top of ""muck"" pile (muddy organic material remaining after deforestation of a swamp). The excavator's tracks unearthed a 15' oak log approximately 10"" in diameter causing it to enter the cab of the machine through the lower front windshield. The operator received a fractured left ankle.

Over-exertion in pulling or pushing objects

Mob. Mech. was dragging fuel hose between tractors when he noticed a burning in the back of his right shoulder running down his tricep. He was seen by co. dr. on 1/12/10 stating he was getting better. Returned to co. dr. 9/15/10 & referred for ortho consult. He saw ortho dr. on 10/4 & was prescribed PT.

Repetitive motion, employee with significant time spent working with his hands.
