Drowning

The victim was operating an excavator near a canal when the excavator tipped on its side and went in the water. The victim was able to get out of the cab and transported to a hospital where he died at 5:45 p.m.

Bodily reaction, (Not Elsewhere Classified)

Employee was preparing to put lock on lock box, he went to turn and walk off and felt a twist in his right knee. Employee returned to company clinic on 4/21/16 with complaint of right knee pain from 2015 incident. Employee being referred for evaluation.

Struck by... (Not Elsewhere Classified)

Float Crew was changing out a 7' long 20"" exp. joint on matrix pump. Bolt holes were not lining up on flanges. Float Crew Leaderman(FCL) used a lifting strap as choker around exp. joint. FCL tightened down on strap & signaled operator to hoist up. As operator started hoisting, the strap broke. FCL left hand was still on strap causing it to suddenly shift.
