Operator was attempting to lean seat forward in pit car. On his first attempt the seat would not adjust. The operator leaned forward to apply more pressure and moved his left hand to the front of the seat with his fingers curled up under the seat. When the seat released the operator's left middle finger was pinched between the seat frame and the adjustment bar.

Struck against stationary object

On 7/28 while trying to extinguish a fire on excavator, employee slipped on track of machine while reaching for handrail striking left shin on tracks. Incident was reported as report only incident. On 8/5(0700)employee presented to site clinic with unresolved leg pain & swelling from incident. The injured employee was evaluated & released to work with sedentary restrictions.

Over-exertion (Not Elsewhere Classified)

Employee was in the process of climbing up onto a dewatering barge in the No. 4 area. While doing so he felt a pop in his left hand, employee felt no immediate pain but approx. 10 minutes later his hand started to hurt. He was treated at company clinic. On 7/12/13 employee was referred to orthopedic institute and placed on light duty.

Over-exertion (Not Elsewhere Classified)

Employee had been using an impact wrench removing bolts. While picking up bolts after finishing the job he experienced discomfort to his left wrist and forearm. Employee's discomfort remained unresolved and was then referred to an orthopedic institute on Monday, June 24 for evaluation and was placed on modified work with no use of left wrist.

Sandblast hose came on for a second, grit impacted face, caused abrasion and impacted left eye.

Remove flex hose from across pipe, IE tossed hose felt pain L shoulder to clinic 7/30 OTC meds.IE treat by co dr 7/31 w/ OTC meds.9/12 IE refer to ortho dr,9/19 dr report no functional IE limit had 0 complaint for months.3/6/14 IE request dr chg,delay appt-w/c adj had difficulty contact IE.5/19 IE saw ortho dr,placed on MW(10#lifting)IE returned to MW on next sch work day,5/23.

Over-exertion (Not Elsewhere Classified)

Employee developed bilateral carpal tunnel syndrome as a result of years of vibratory tool use. Employee received diagnosis from personal medical doctor both wrists on March 21, left wrist worse. On March 25 employee had a non-work related ganglion cyst removed and also had left CT released without employer knowledge. Employee returned to work on May 6, 2013.
